摘　　要：目的分析神经梳理术治疗无明确责任血管型三叉神经痛的疗效及手术安全性。方法2011年至2014年对23例术中证实无明确责任血管的三叉神经痛患者行三叉神经感觉根梳理术。结果术后即刻疼痛完全缓解率为78%,总有效率为96%,无效1例。随访2~5年,疼痛复发4例。所有患者术后均有轻至中度面部麻木,多数在术后3个月内明显减轻,5例面部遗留轻度麻木。无听力下降、面瘫、脑脊液漏、颅内感染或出血等并发症。结论神经梳理术可作为无明确血管压迫型三叉神经痛的一种安全、有效的治疗手段。Objective The effects and security of nerve combing for trigeminal neuralgia without vascular compression were evaluated. Methods A total of 23 trigeminal neuragial patients without definite neurovascular compression before operation, who were treated with microvascular decompression in our department, were performed the nerve combining. Results Post-operative immediate complete remission rate was 78%, with a total effective rate of 96%. All the patients had been followed up for 2 - 5 years, with 4 suffering pain relapses. Most patients experienced mild to moderate numbness, which was gradually relieved and did not significantly affect the life quality. Permanent mild and tolerable numbness were presented in 4 patients. No hearing loss, facial paralysis, CSF leakage and infection or other serious complications were observed. Conclusion Nerve combing is a safe and effective treatment for trigemiual neuralgia in the absence of neurovascular compression.
