经皮附睾穿刺取精术联合睾丸穿刺取精术在评估睾丸生精功能中的应用价值

摘　　要：目的探讨经皮附睾穿刺取精术(PESA)联合经皮附睾穿刺取精术(TESA)在评估无精子症患者睾丸生精功能中的应用价值。方法 394例无精子症患者均行睾丸活检术以及PESA联合TESA,评估患者睾丸生精功能的效果。结果 394例患者中PESA联合TESA发现精子232例,未见精子162例;睾丸活检术发现精子233例,未见精子161例。PESA联合TESA评估睾丸生精功能的灵敏度、特异度、准确率分别为99.57%(232/233)、100.00%(161/161)、99.75%(393/394)。PESA联合TESA、睾丸活检术的精子获取率分别为58.88%(232/394)和59.14%(233/394),两者比较,差异无统计学意义(P>0.05)。结论 PESA联合TESA能较准确地评估无精子症患者的睾丸生精功能。Objective To explore the value of percutaneous epididymal sperm aspiration （ PESA） combined with testicular sperm aspiration（ TESA） in evaluating testicular spermatogenic function of patients with azoospermia .Methods A total of 394 patients with azoospermia underwent testicular biopsy and the combination examination （ PESA and TESA ） .And the value of the combination method for evaluating testicular spermatogenic function was assessed using the biopsy result as golden standard .Rseults Of 394 patients,sperms were observed in 232 cases and no sperms in 162 cases by combination of PESA with TESA .Sperms were observed in 233 cases and no sperms in 161 cases by testicular biopsy.The sensitivity,specificity and accuracy rate of the combination method were 99.57%（232/233）, 100.00%（161/161）,and 99.75%（393/394）,respectively.The sperm harvesting rates of the combination method and testicular biopsy were 58.88%and 59.14%,respectively,and there was no statistical difference between the two methods （P〉0.05）.Conclusion The combination of PESA with TESA can evaluate the testicular spermatogenic function of patients with azoospermia quite accurately .
