摘  要:目的:研究50%氙气后处理对兔脊髓缺血再灌注损伤的保护作用和时间窗效应。方法:54只新西兰白兔采用随机法分为9组,每组6只。首先应用腹主动脉球囊阻断法建立兔脊髓缺血再灌注损伤模型(I/R),然后,根据再灌注后不同时间点进行氙气后处理分为:(1)I/R空白组;(2)即刻氙气后处理组(T0h);(3)延迟1h氙气后处理组(T1h);(4)延迟2h氙气后处理组(T2h);(5)延迟3h氙气后处理组(T3h);(6)延迟7h氙气后处理组(T7h组);(7)延迟23h氙气后处理组(T23h);(8)延迟47h氙气后处理组(T47h);(9)假手术组(Sham)。各实验组动物均于再灌注后不同时刻经吸气装置吸入50%氮气+50%氧气混合气,持续1h,期间持续监测氧气及二氧化碳浓度。分别于缺血再灌注后4h、8h、24h、48h和72h观察记录兔后肢运动功能。于缺血再灌注后72h取脊髓组织行HE染色、TUNEL染色检测正常脊髓前角神经元和凋亡前角运动神经元数目。结果:与Sham组相比,I/R组及各实验组运动评分随时间逐渐下降(P<0.001)。T2h和T3h组在各时间点运动评分均大于I/R组(P<0.05);在缺血再灌注损伤24,48和72h,T2h组和T3h组运动评分大于I/R组和T47h组(P<0.05);在缺血再灌注损伤48h,T2h组和T3h组运动评分高于T0h和T1h组(P<0.05)。与Sham组相比,I/R组、T0h组、T1h、T7h组、T23h组和T47h组正常脊髓前角运动神经元数目的减少(P<0.05);与I/R组相比,T2h组和T3h组的正常脊髓前角运动神经元数目显著增多(P<0.05)。与Sham组相比,I/R组及各T组均出现神经元和神经胶质细胞凋亡;与I/R组相比,T0h组、T1h组凋亡脊髓前角运动神经元数目较少(P<0.05)。结论:氙气后处理对兔脊髓缺血再灌注损伤引起的运动功能障碍有显著保护作用。兔脊髓缺血再灌注后2~3h行氙气后处理的脊髓保护效果最佳。Objective:To explore the protective roll and the time window of 50% xenon (Xe) post-con- ditioning on motor function of spinal cord following ischemia/reperfusion injury (SCIRI). Methods : A total of 54 male rabbits were randomLy divided into 9 groups averagely: ( 1 ) Sham group, ( 2 ) I/R group, ( 3 ) post-con- ditioning immediately ( T0h Group) , ( 4 ) 1 h delayed post-conditioning( T1 h Group) , ( 5 ) 2h delayed post-con- ditioning(T2h Group), (6) 3h delayed post-conditioning (T3h Group), (7) 7h delayed post-conditioning ( TTh Group), ( 8 ) 23 h delayed post-conditioning ( T23 h Group ), ( 9 ) 47h delayed post-conditioning ( T47h Group). Motor function of hindlimb were graded by 4h,8h,24h,48h and 72h after I/R. Counting the number of normal motor neurons and apoptotic motor neurons of spinal by H&E staining and TUNEL staining. Results: Compared with Sham group, all other groups showed remarkable time related decrease of hindlimb motor func- tion, (P 〈0.001 ). The T2h group and T3h group has higher grade than I/R group at each grading time point (P 〈 0.05 ). The T2h group and T3h group has higher grade than I/R group and T47h group after 24 hours post-reperfusion(P 〈 0.05 ). The T2h group and T3h group have higher grade than T0h group and Tlh group after 24 hours post-reperfusion ( P 〈 0.05 ). Compared with Sham group, I/R group, T0h group, Tlh group, TTh group, T23h group and T47 group have fewer normal motor neurons ( P 〈 0.05 ). Compared with L/R group, T2h group and T3h group have more normal motor neurons (P 〈0, 05 ). Compared with Sham group, all other groups has apoptotic neurons. TOh group and Tlh group have fewer normal motor neurons than I/R group ( P 〈0. 05 ). Conduslon: Xenon has significant protective effects on the motor function injury following by spi- nal cord ischemia/reperfusion.
