摘 要:目的了解新疆志贺氏菌菌型及对23种抗菌药物的耐药情况,为痢疾的防治和临床用药提供科学依据。方法对2006─2015年新疆各肠道传染病监测点现场分离的2 907株志贺氏菌进行系统分型鉴定,并挑选部分菌株(2 215株)采用琼脂扩散法(K-B法)检测对23种抗菌药物的敏感性。结果 2 907株受试菌菌群分布以福氏志贺氏菌居优势、占91.88%,其次为宋内志贺氏菌(6.71%)、痢疾志贺氏菌(0.93%)和鲍氏志贺氏菌(0.48%);占优势的福氏志贺氏菌亚型分布除2014年F2b*(42.86%)为优势外,其他年份均以F2a为主,各监测点无明显差异;2 215株菌株对23种抗菌药均有不同程度耐药,耐药率居前3位的依次为青霉素(94.36%)、链霉素(89.71%)和强力霉素(87.81%),没有100.00%敏感的抗菌药物,并且耐受6种及以上抗菌药物的菌株比例较高为57.34%。结论新疆志贺氏菌以福氏属中的F2a为优势株,细菌性痢疾是新疆常见而多发的肠道传染病之一,对常用抗菌药物耐药情况较为严重,因此正确选用抗菌药物是有效治疗痢疾的关键。Objective To understand serotype distribution and drug resistance of Shigella in Xingjiang from 2006 to 2015,to provide scientific basis for prevention and clinical medication of diarrhea. Methods Two thousand, nine hundred and seven stains of Shigella isolated from the intestinal infectious disease monitoring points in Xingjiang during 2006 to 2015 were detected for serotype of Shigella, and 2 215 stains were selected to determine sensitivity to 23 kinds of antibiotics with K-B method. Results The main serotype was Shigella flexneri, accounting for 91.88%, followed by Shigella sonnei(6.71%), Shigella dysenteriae(0.93%) and Shigella boydii(0.48%). The main serotype was F2 a, except for f2b*in 2014(42.86%), without obvious difference among the monitoring points. There was drug resistance to 23 kinds of antibiotics for the 2 215 stains of Shigella, with the top three resistance rate of penicillin(94.36%), streptomycin(89.71%) and doxycycline(87.81%). 57.34% of the stains were even drug resistant to six or more than six kinds of antibiotics. Conclusions Flexneri F2 a is the main serotype of Shigella stains in Xingjiang during 2006 to 2015. Bacillary dysentery is one of the most intestinal infectious diseases in Xingjiang, and severely resistant to commonly used antibiotics. Sufficient attention should be paid to using appropriate antibiotic to cure bacilliry dysentery.
