输血科参与临床会诊的模式探讨

摘　　要：目的:探讨输血科参与临床会诊的方向和模式。方法:查阅我科近5年参与临床会诊的病例,从会诊科室的分布,会诊例次,会诊目的及会诊的临床效果进行回顾性分析。结果:近5年参与临床会诊总数为53例,会诊科室为15个科室,其中内科为5个科室,占33.3%,外科为10个科室,占66.7%。会诊科室最多的为妇产科,会诊15例,占28.3%。从会诊的目的看,技术指导占17.0%;安全用血评估占35.8%;多学科联合会诊占47.2%。从会诊的效果看,临床采纳输血科建议率为96.2%。发生输血不良反应及输血引起的相关疾病为零。结论:输血科积极参与临床会诊,为临床科学﹑安全﹑有效地用血提供有力保障,能有效地降低输血风险。Objective：To study the transfusion direction and pattern of the participation in clinical consultation. Methods： The participation cases in clinical consultation of our department in recent 5 years were collected, and the consultation department, the distribution of consultation cases, consultation purpose and the clinical effect of con- sultation were retrospectively analyzed. Results： The total number of nearly 5 years in clinical consultation was 53 cases, consultation department was 15 departments, including internal medicine for 5 departments, accounting for 33.3%, surgery for 10 departments, accounting for 66.7%. The most clinical consultation department was gyne- cology and obstetrics department, with a total of 15 cases （28.3%）. Looked from the purpose of the consultation, technical guidance accounted for 17.0%, safety assessment of blood accounted for 35.8%, and multidisciplinary collaborative diagnosis accounted for 47.2%. From the effect of consultation, the adoption rate of clinical blood transfusion department recommendation was 96.2 %. The incidence of blood transfusion adverse reactions and re- lated disease caused by transfusion was zero. Conclusion： Blood transfusion actively participating in clinical consul- tation would provide powerful guarantee for effective, scientifical and safe clinical blood use, and effectively reduce the risk of blood transfusion.
