摘 要:目的探讨应用脊柱内窥镜技术(TESSYS)治疗腰椎间盘突出症(LDH)对降低患者疼痛及复发的效果。方法 39例腰椎间盘突出症患者作为观察对象,全部患者应用脊柱内窥镜技术治疗。采用Oswestry功能障碍指数问卷表(ODI)评分及视觉模拟评分法(VAS)评分对患者术前、术后及术后1、3、6、12个月的状态进行评估。随访中采用改良Mac Nab功能评价标准评定疗效。结果患者术后及术后1、3、6、12个月的VAS评分和ODI评分均低于术前,差异均具有统计学意义(P<0.05)。随访并以改良Mac Nab功能评价标准评定术后疗效,术后1个月优良率为82.05%、3个月为87.18%、6个月为92.31%、12个月为97.14%,且均未见复发情况出现。结论脊柱内窥镜技术具有安全性高、疗效确切、并发症少、术后优良率高及复发率低的特点,达到一定程度的微创效果,弥补了传统手术的不足,在腰椎间盘突出症的治疗中运用价值极高,值得推广。Objective To explore the effect of lumbar disc herniation(LDH) treated with transforaminal endoscopic spine system(TESSYS) in reducing pain and recurrence. Methods A total of 39 lumbar disc herniation patients as observation subjects and they were all received transforaminal endoscopic spine system for treatment. State of patients in preoperation, postoperation and 1, 3, 6, 12 months postoperation was evaluated by Oswestry disability index(ODI) score and visual analogue scale(VAS) score. Curative effect was evaluated by improved Mac Nab function evaluation standard in follow-up review. Results Patients had lower VAS and ODI score in postoperation and 1, 3, 6, 12 months postoperation than preoperation, and the difference had statistical significance(P0.05). Postoperative effect was evaluated by improved Mac Nab function evaluation standard in follow-up reviews, with good-excellent rate as 82.05% in 1 month postoperation, 87.18% in 3 months postoperation, 92.31% in 6 months postoperation and 97.14% in 12 months in 12 months postoperation, and no recurrence appeared. Conclusion Transforaminal endoscopic spine system has characteristics of high safety, affirmative curative effect, less complications, high postoperative good-excellence rate and low recurrence rate. It has minimally invasive effect to a certain degree, and make up for the deficiencies of traditional surgery, so it has high application value in the treatment of lumbar disc herniation, and it is worthy of promotion.
