组织多普勒成像技术定量房室瓣环运动在高血压心脏病左心室舒张功能评价中的应用

摘　　要：目的采用组织多普勒成像(DTI)技术对高血压心脏病患者的左心室功能变化进行评价。方法 75例高血压心脏病患者作为高血压心脏病组,20例健康体检者作为健康对照组,采用DTI技术及多普勒血流频谱测量舒张早期充盈峰(E)及舒张晚期充盈峰(A)、舒张早期运动速度(Ea)及舒张晚期运动速度(Aa),并进行组间对照研究。结果高血压心脏病组Ea(7.90±2.50)cm/s、Aa(9.30±2.60)cm/s及Ea/Aa值(0.84±0.05)均低于健康对照组(14.80±5.30)cm/s、(12.50±2.30)cm/s、(1.18±0.34),差异均具有统计学意义(P<0.05)。高血压心脏病组E(64.90±19.71)cm/s、A(75.03±18.42)cm/s及E/A值(0.91±0.21)均低于健康对照组(89.33±18.86)cm/s、(87.65±22.80)cm/s、(1.19±0.14),差异均具有统计学意义(P<0.05)。结论 DTI技术结合多普勒血流频谱测定可更为有效的对高血压心脏病左心室舒张功能进行评价。Objective To evaluate the left ventricular diastolic function changes in hypertension and heart disease patients by Doppler tissue imaging（DTI） technique. Methods A total of 75 hypertension and heart disease patients as hypertension and heart disease group, and 20 health examination people as health control group. Measurement were made on early diastolic filling peak（E）, late diastolic peak（A）, early diastolic filling velocity（Ea） and late diastolic velocity（Aa） by DTI technique and Doppler flow spectrum for contrast in two groups. Results The hypertension and heart disease patients had Ea as（7.90±2.50） cm/s, Aa as（9.30±2.60） cm/s, and Ea/Aa value as（0.84±0.05）, which were all lower than（14.80±5.30） cm/s,（12.50±2.30） cm/s and（1.18±0.34） in the health control group, and their differences had statistical significance（P0.05）. The hypertension and heart disease patients had E as（64.90±19.71） cm/s, A as（75.03±18.42） cm/s, E/A value as（0.91±0.21）, which were all lower（89.33±18.86） cm/s,（87.65±22.80） cm/s and（1.19±0.14）, and their differences had statistical significance（P0.05）. Conclusion Combination of DTI technique and Doppler blood flow spectrum measurement can effectively evaluate the left ventricular diastolic function in hypertension and heart disease.
