摘　　要：目的探讨N-乙酰半胱氨酸(N-acetylcysteine,NAC)联合肺康复对类风湿关节炎相关间质性肺病(Rheumatoid arthritis-interstitial lung disease,RA-ILD)患者的运动耐力及生活质量的影响。方法筛选RA-ILD患者48例,随机分为A组(常规药物治疗组)、B组(NAC组)、C组(NAC联合肺康复组),每组16例,均接受常规药物治疗。治疗前及治疗后12周,均进行6分钟步行距离(6-minute walk distance,6MWD)、圣乔治呼吸问卷调查(SGRQ)评分及肺功能[第1秒用力呼气容积(FEV1)占预计值百分比(FEV1%pred)、百分肺活量(VC%)、单位肺泡容积的一氧化碳弥散量占预计值百分比(DLCO%)]测定。结果各组患者治疗前6MWD、SGRQ评分及肺功能指标比较差异无统计学意义(P>0.05)。治疗后,A组各项指标与治疗前相比差异无统计学意义(P>0.05),B组、C组6MWD及肺功能指标升高(P<0.05),而SGRQ评分降低(P<0.05),且C组6MWD、SGRQ评分及肺功能指标(FEV1%pred、VC%)优于B组(P<0.05)。结论 NAC联合肺康复治疗对改善RA-ILD患者的运动耐力及生活质量有显著疗效。Objective To explore the clinical effect of N-acetylcysteine（NAC） and pulmonary rehabilitation in the treatment of the exercise endurance and quality of life of patients with rheumatoid arthritis-interstitial lung disease（RA-ILD）.Methods Totally 48 RA-ILD patients were randomly divided into three groups：conventional drug treatment group（group A）,NAC treatment group（group B） and NAC ＋ pulmonary rehabilitation treatment group（group C）.Each group consisted of 16 cases.Six-minute walk distance（6MWD）,St.George＇s Respiratory Questionnaire（SGRQ） and lung function were determined before and after treatment.Results There was no significant difference in6 MWD,SGRQ,FEV1%pred,VC%and DLCO%pred among the three groups before treatment（P0.05）.After treatment,all the indicators of group A showed no significant change（P0.05）;the 6MWD and lung function in group B and group C increased（P0.05）,while the SGRQ score decreased（P0.05）,and there were significant differences in 6MWD,SGRQ scores and pulmonary function（FEV1%pred and VC%） between group C and group B（P0.05）.Conclusion NAC combined with pulmonary rehabilitation can significantly improve the exercise endurance and quality of life of RA-LLD patients.
