摘　　要：[目的]研究过渡期护理模式在冠心病病人从医院到家庭过渡期中的应用效果。[方法]将我科255例冠心病病人随机分为对照组125例和观察组130例,对照组按常规方法实施照护,观察组除常规方法外根据病人不同情况制定针对性的护理干预措施,实施过渡期护理照护方案。出院后30d比较两组病人的自我管理行为、自我效能、生活质量、依从性、再次门急诊率、再入院率、满意度。[结果]出院后30d两组病人自我管理行为、自我效能、依从性、满意度得分及再次门急诊率、再入院率比较差异有统计学意义(P<0.01);两组简明健康问卷(SF-36)各维度得分比较差异均有统计学意义(P<0.01)。[结论]对冠心病病人实施过渡期护理照护,可明显提高病人的自我管理行为、自我效能及生活质量,提高病人的依从性及满意度,减少再次门急诊率和再入院率。Objective：To probe into the application effect of transitional nursing model in transitional period of patients with coronary heart disease from hospital to family.Methods：A total of 255 coronary heart disease patients in our department were randomly divided into control group of 125 case and observation group of 130 case.Patients in control group was treated with routine care,patients in observation group in addition to conventional methods based on patients with different circumstances to develop targeted nursing interventions,the implementation of transitional care program.The self-management behavior,self-efficacy,quality of life,compliance,re emergency rate,readmission rate,and satisfaction of the patients in the two groups were compared in 30 day of patients leave hospital.Results：There was significant difference in self-management behavior,self-efficacy,compliance,satisfaction score and re-hospital emergency rate and readmission rate between two groups in 30 day after discharge（P〈0.01）;There was significant difference in concise Health Questionnaire（SF-36）all dimensions score between two groups（P〈0.01）.Conclusions：The implementation of the transition period of nursing care for patients with coronary heart disease could improve the patient＇s self-management behavior,self-efficacy and quality of life,improve patient compliance and satisfaction,reduce the re-emergency rate and readmission rate.
