不同胰肠吻合方式在PD术中的临床应用比较

摘　　要：目的探讨并分析胰管-空肠黏膜吻合术和捆绑式胰肠套入吻合术在胰十二指肠切除术中的临床应用效果。方法回顾性研究我院近五年行胰十二指肠术患者57例,其中胰管-空肠黏膜吻合式31例,捆绑式胰肠套入吻合式26例,收集患者术前一般情况及术中术后各项指标,分析两种方式有无差异。结果捆绑式胰肠套入吻合较胰管-空肠黏膜吻合手术时间缩短(358.7±7.8 min vs 307.4±5.8 min,P<0.05),术后主要并发症胰瘘、腹腔出血以及住院时间两组无统计学差异。结论胰管-空肠黏膜吻合和捆绑式胰肠套入吻合均具有较好的临床效果,捆绑式胰肠套入吻合较胰管-空肠黏膜吻合节省手术时间,具体吻合方式因根据个人习惯和熟练程度灵活选择。Objective To explore and analyze the effect of clinical application of duct-to-mucosa pancreaticojejunostomy andbinding pancreaticojejunostomy in Pancreaticoduodenectomy. Methods The clinical data of 57 patient who underwent pancreaticoduo- denectomy at our hospital in the last five years were collected and retrospective studied, of all patients, 31 underwent duct-to-mucosa pancreaticojejunostomy and 26 underwent binding pancreaticoje -junostomy, the surgical and postoperative indicators were collected to analyze whether there is any difference between the two methods. Results The mean time to complete binding pancreaticojejunostomy is shorter than duct-to-mucosa pancreaticojejunostomy （358. 7 ± 7.8 min vs 307.4 ± 5.8 min, P 〈 0. 05 ）, no significant differences were noted in pancreatic fistula, abdominal bleeding and hospitalization time among the two groups. Conclusions Satisfactory efficacy can be achieved using any of the two methods of pancreaticojejunostomy, binding pancreaticojejunostomy has shorter operative time than the other method, we should choose the specific method according to our habits and familiarity.
