HPV感染基因型与宫颈鳞癌致癌性关系的多中心回顾性研究

摘　　要：目的探讨宫颈鳞状细胞癌组织中人乳头瘤病毒(HPV)感染基因型与致癌性的关系。方法来自一项多中心的临床研究的宫颈鳞状细胞癌组织标本653例,使用膜杂交芯片技术行23种HPV分型检测,并对患者的相关资料进行分析。结果宫颈鳞状细胞癌组织标本中,总的HPV感染率为90.05%(588/653),其中一型感染率为73.97%(483/653),多型感染率为16.08%(105/653)。宫颈鳞状细胞癌组织中,HPV一型感染是主流,多型感染是支流,HPV感染最主要的高风险型别依次为16、18、58、33、31、52、59、45、66、73型,16型最多,占高危型出现频率的60.90%(422/693),16型和18型占高危型出现频率的71.00%(492/693)。结论 HPV基因膜芯片检测技术可用于宫颈鳞状细胞癌组织的分型检测,对我国女性宫颈鳞状细胞癌的防治、HPV疫苗的研发以及宫颈鳞状细胞癌组织HPV感染分型数据库的建立都具有重要的意义。Objective To explore the relationship between human papillomavirus（HPV） infection genotypes and cervical squamous cell carcinoma.Methods A total of 653 cervical squamous cell carcinoma specimens from a multicenter clinical study were selected,23 kinds of HPV genotypes were detected by membrane hybridization chip technology,the related data of the patients were analyzed.Results The total infection rate of HPV was 90.05%（588/653）,the single and multiple HPV genotype infection rates were 73.97%（483/653） and 16.08%（105/653）,respectively.The main HPV infection type was multiple HPV genotype infection.The high-risk HPV infection genotypes were HPV 16,HPV 18,HPV 58,HPV 33,HPV 31,HPV 52,HPV 59,HPV 45,HPV 66,HPV 73,the percentage of HPV 16 was the highest,accounting for 60.90%（422/693）,while the percentage of HPV 16 combined with HPV 18 accounted for71.00%（492/693）.Conclusion HPV membrane hybridization chip technology can be used to detect and classify cervical squamous cell carcinoma,which is of great significance for prevention and treatment of cervical squamous cell carcinoma,research of HPV vaccine,and establishment of HPV infection genotypes database in cervical squamous cell carcinoma patients.
