二甲双胍联合炔雌醇环丙孕酮治疗多囊卵巢综合征不孕症的临床观察

摘　　要：目的分析二甲双胍联合炔雌醇环丙孕酮治疗多囊卵巢综合征排卵障碍性不孕的临床疗效。方法选取多囊卵巢综合征排卵障碍性不孕患者71例,行前瞻性分组对照研究,采取双盲随机法将患者划分为两组,其中35例采取炔雌醇环丙孕酮进行治疗,设置为对照组,其余36例则在炔雌醇环丙孕酮的基础上结合二甲双胍进行治疗,设置为观察组;对比两组患者治疗3~6个月的临床疗效。结果两组患者治疗后各项激素指标均得到明显改善,观察组LH(7.13±2.27)U/L、T(1.59±0.53)mmol/L明显优于对照组的LH(8.67±3.01)U/L、T(2.24±0.85)mmol/L,差异具有统计学意义(P<0.05),观察组患者治疗总有效率(91.67%)、排卵率(80.56%)以及成功妊娠率(41.67%)均显著高于对照组(P<0.05)。结论采用二甲双胍联合炔雌醇环丙孕酮治疗多囊卵巢综合征排卵障碍性不孕可迅速改善患者激素水平,增加排卵与妊娠成功率,其药性温和无明显并发症,安全有效,值得临床应用。Objective To analyze the clinical effect of metformin combined with ethinylestradiol/cyproterone acetate in treatment of infertility due to polycystic ovary syndrome（PCOS）.Methods Seventy-one infertile patients due to PCOS were selected,then a prospective grouping control study was performed.The patients were divided into two groups according to random double-blind method,35 patients in control group were treated by ethinylestradiol/cyproterone acetate,and 36 patients in observation group were treated by metformin combined with ethinylestradiol/cyproterone acetate.The clinical curative effects at 3-6 months after treatment in the two groups were compared.Results After treatment,hormone indexes were improved significantly in the two groups.The levels of luteinizing hormone（LH） and testosterone（T） in observation group were（7.13±2.27） U/L and（1.59±0.53） mmol/L,respectively,which were statistically significantly better than those in control group [（8.67±3.01） U/L and（2.24±0.85） mmol/L]（P〈0.05）.The total effective rate,ovulation rate,and successful pregnancy rate in observation group were 91.67%,80.56%,and 41.67%,respectively,which were statistically significantly higher than those in control group（P〈0.05）.Conclusion Metformin combined with ethinylestradiol/cyproterone acetate can rapidly improve hormone levels of the infertile patients due to PCOS,increase ovulation rate and pregnancy successful rate,the method is mild,safe and effective,and it has no obvious complications,which is worthy of clinical application.
