强直性脊柱炎骶髂关节低场强磁共振成像与患者血清白细胞介素-18肿瘤坏死因子-α水平的相关性分析

摘　　要：目的探讨强直性脊柱炎(AS)骶髂关节低场强磁共振成像(MRI)与患者血清白细胞介素(IL)-18、肿瘤坏死因子(TNF)-α水平的相关性。方法收集2012年1月至2015年3月来我院就诊的AS患者150例骶髂关节,同期收集体检健康的志愿者50名,分别给予低场强MRI和血清IL-18、TNF-α水平检测,分析其相关性。结果AS患病组骶髂关节经低场强MRI检查,累及范围计分、水肿强度计分、水肿深度计分和总评分及血清IL-18、TNF-α水平均高于健康组,差异均有统计学意义(P<0.05)。同时采用Logistic回归进行相关性分析,上述评分差异均有统计学意义(P<0.05),且各项指标最大似然估计值和比值比(OR)值均大于0。结论 AS骶髂关节低场强MRI与患者血清IL-18、TNF-α水平呈正相关。Objective To investigate the correlation between the level of serum interleukin-18 （IL-18） and tumor necrosis factor-or （TNF-α） and low field magnetic resonance imaging and sacral joint of ankylosing spondylitis （AS）. Methods One hundred and fifty cases of patients with AS of sacral joint accepting treatment in our hospital were collected from January 2012 to March 2015. Fifty cases of healthy volunteers were collected during the same period. Low field magnetic resonance imaging and serum IL-18 and TNF-α were detected respectively, and their correlation was analyzed. Results The sacral joint of patients with AS were examined by low field magnetic resonance imaging, and the scores of the patients, the score of the knee, the intensity of the edema, the depth of the edema, the total score and the serum IL-18 and TNF-α were higher than those in the healthy group. The difference was statistically significant （P〈O.05）. Using Logistic regression analysis, the difference was statistically significant （P〈0.05）, and the maximum likelihood estimates and OR values of the indicators were more than O. Conclusion There is a positive correlation between the serum levels of IL-18 and TNF-α and low field magnetic resonance imaging and sacral joint of AS.
