深低温冻存自体血小板输注治疗恶性血液病化疗后血小板减少症的疗效和安全性  

Efficacy and safety of cryopreserved autologous platelet transfusion in the treatment of thrombocytopenia after chemotherapy for hematological malignancy

摘　　要：目的探讨恶性血液病患者输注深低温冻存的自体血小板后对化疗后血小板减少症的影响。方法选择2014年11月~2016年11月我院采用化疗方法治疗恶性血液病并发生血小板减少症的80例患者,按照选择的不同血小板输注来源分组:输注经过深低温冻存的自体血小板的40例患者为研究组,皮下注射抗人重组白细胞介素11(rhIL-11)的40例患者为对照组。对比两组患者的治疗效果与不良反应率。结果研究组的有效率为97.5%,对照组为85.0%,研究组的疗效优于对照组;研究组的不良反应率为2.5%,对照组为17.5%,研究组的不良反应率低于对照组,差异有统计学意义(P<0.05)。研究组治疗前的血小板水平为(51.58±16.34)×10~9/L,治疗后的血小板水平为(68.53±42.15)×10~9/L;对照组治疗前的血小板水平为(51.39±17.05)×10~9/L,治疗后的血小板水平为(57.05±22.72)×10~9/L,两组的血小板水平治疗前差异无统计学意义(P>0.05),治疗后两组的血小板水平有所改变,且研究组较对照组改变更明显,差异有统计学意义(P<0.05)。结论为恶性血液病患者在化疗后输注经过深低温冻存的自体血小板,可以有效缓解其血小板减少症状,同时不会引发严重不良反应。Objective To investigate the effect of cryopreserved autologous platelets transfusion in the patients with hematological malignancy on the reduction of platelets after chemotherapy. Methods From November ：2014 to November 2016, 80 patients with hematological malignancy complicated with thrombocytopenia undergoing chemotherapy in our hospital were selected. According tO the different selection of platelet transfusion sources, 40 patients who underwent cryopreserved autologous platelets transfusion were assigned to the study group, and 40 patients who underwent subcutaneous injection of anti-human recombinant interleukin-11（rhIL-11） were assigned to the control group. The therapeutic effect and adverse reaction rate were compared between the two groups. Results In terms of the efficiency, the study group was 97.5% and the control group was 85.0%. In terms of adverse reaction rate, the study group was 2.5% and the control group was 17.5%, the study group was lower than the control group, and the difference was statistically signifi cant （P〈0.05）. In addition, the level of platelets before treatment in the study group was（51.58±16.34）×10^9/L, and the platelet level after treatment was（68.53±42.15）×10^9/L; the level of platelets before treatment in the control group was （51.39±17.05）×10^9/L, and the levels of platelet after treatment was （57.05±22.72）×10^9/L. The platelet level in the two groups of patients did not show statistically significant difference before the treatment（P〉0.05）, were both changed after the treatment, and the changes in the study group were significantly greater than those in the control group, the differ ences were statistieally significant（P〈0.05）. Conclusion Transfusion of cryopreserved autologous platelets after the chemotherapy in the patients with hematological malignancy can effectively relieve the symptoms of thromboeytopenia without causing serious adverse reactions.
