难治性产后出血实施子宫填塞球囊导管联合宫颈钳夹术的治疗效果

摘　　要：目的探讨和分析难治性产后出血实施Bakri子宫填塞球囊导管联合宫颈钳夹术的治疗效果。方法选取我院在2015年7月~2016年6月收治的79例难治性产后出血患者,按照入院顺序分为观察组42例和对照组37例。两组患者均采用Bakri子宫填塞球囊导管填塞宫腔进行治疗,观察组患者加用宫颈钳夹术进行治疗,比较两组患者的子宫切除情况、止血效果以及出血量。结果观察组患者的子宫切除率(0.00%)低于对照组(10.81%),止血有效率(97.62%)高于对照组(83.78%),出血量[(146.92±13.25)ml]少于对照组[(269.25±18.25)]ml,差异均有统计学意义(χ~2=4.783,P=0.029;χ~2=4.663,P=0.031;t=34.369,P=0.000)。结论难治性产后出血实施Bakri子宫填塞球囊导管联合宫颈钳夹术的治疗效果确切,能有效降低患者子宫切除风险,提高止血有效率,减少出血量,值得推广。Objective To explore and analyze the efficacy of uterine tamponade balloon catheter combined with cervical clamp in intractable postpartum hemorrhage.Methods 79 patients diagnosed with intractable postpartum hemorrhagein our hospital from July 2015 to June 2016 were selected and divided into the observation group （42 cases） and the con- trol group （37cases） according to the hospital admission sequence.All patients in two groups were received uterine tam- ponade balloon catheter and patients in the observation group were also received cervical clamp in addition,the number of hysterectomy,hemostatic effect and amount of bleeding of patients between two groups were conpared.Results In the observation group,the incidence of hysterectomy of patients （0.00%） was lower than that in the control group （10.81%）, the success rate of hemostasis （97.62%） was higher than that in the control group（83.78%）;the amount of bleeding [（146.92± 13.25）ml] was less than that in the control group [（269.25±18.25）ml],and the differences were statistically significant （X2=4.783,P=-0.029;X2=4.663,P=0.031;t=34.369,P=0.000）.Conclusion The efficacy of uterine tamponade balloon catheter combined with cervical clamp in intractable postpartum hemorrhage is accurate,which can reduce the incidence of hysterec- tomy,increase the success rate of hemostasis and reduce intraoperative hemorrhage,and worthy of clinical promotion.
