抗SSA、抗SSB、抗Ro52抗体在自身免疫性疾病中的临床价值探讨

摘　　要：目的研究抗SSA、抗SSB和抗Ro52抗体在自身免疫性疾病(autoimmune diseases,AID)诊断中的临床意义,探讨在SLE、SS、RA中相关实验室项目结果在抗SSA/SSB/Ro52抗体的单阳性组和多阳性组间是否存在差异。方法选取抗SSA/SSB/Ro52抗体结果为阳性的患者,分析他们的相关实验室检测结果与临床最终诊断。结果抗SSA/SSB/Ro52抗体不同阳性组合之间AID和非AID诊断的构成比具有显著差异,并且随着抗体阳性的增多AID所占的百分比也随着增加,各组百分比按大小排列为SSA+SSB+Ro52+>SSA+SSB+/SSA+Ro52+/SSB+R052+>SSA+/SSB+/Ro52+;SLE患者Ig G的升高、RA淋巴细胞的降低、SS患者球蛋白Ig G的升高以及C3的降低在单阳性组与多阳性组间有显著差异。结论抗SSA、抗SSB、抗Ro52抗体可作为AID的筛选指标,阳性抗体越多AID确诊的可能性越大。在SLE、RA、SS患者中,部分实验结果在单阳性组与多阳性组间有显著差异,提示多抗体阳性可能与疾病的活动性或严重程度有关。This study performed to investigate the clinical significance of anti-SSA, anti-SSB and anti-Ro52 antibodies in the diagnosis of autoimmune diseases（AID）, and to investigate whether the results of related laboratorytest in SLE, SS, RA were different between the single positive group and the multiple positive group of anti-SSA/SSB/Ro52 antibodies. Total of 1 007 patients with positive results of SSA/SSB/Ro52 antibody were selected andtheir laboratory test results and clinical final diagnosis were analyzed. Data showed that there was a significantdifference in the proportions of AID and non-AID diagnosis among the different combinations of anti-SSA/SSB/Ro52 antibodies, and the percentage of AID increased with the increase of positive antibody. Moreover, the increaseof Ig G in SLE patients, the reduction of lymphocytes in RA patients, the decrease of C3 and the elevation of globulinIg G in SS patients were significantly different between the single positive group and the multiple positive group. Inconclusion, anti-SSA, anti-SSB, anti-Ro52 antibodies can be used as the screening indexes of AID; the morepositive antibody is, the more likely the diagnosis of AID is, suggesting that multiple positive antibodies may berelated to the activity or severity of diseases.
