摘 要:目的了解登革热疫点健康人群感染状况,为预防及控制登革热提供依据。方法在江门市江海区礼乐街道2014年曾发生登革热疫情的4个疫点随机抽取245名身体健康、2周内无发热史且无登革热病史的本地居民,采集血清标本并用酶联免疫法检测血清中抗登革病毒IgG抗体,与非疫点健康人群的血清标本进行对比。在疫点开展问卷调查,调查登革热感染因素,并进行非条件Logistic回归分析。结果登革热疫点健康人群抗登革病毒IgG抗体阳性率24.49%(60/245),非疫点抗体阳性率3.09%(13/421),两者差异具有统计学意义(X^2=72.69,P<0.001)。抗体阳性率随着年龄的上升而下降(趋势检验X^2=7.07,P<0.05)。经过单因素和多因素非条件Logistic回归分析,影响登革热IgG抗体阳性率的因素有:蚊帐的使用,水生植物的种养和家中环境。结论登革热暴发疫点健康人群感染比例较高,需要针对高危因素预防和控制登革热的流行。Objective To examine dengue fever epidemic infection status in healthy population and to provide evidence for the prevention and control of dengue fever. Methods Epidemiological characteristic analysis of epidemic situation of DF in 2014 was conducted. In addition, a total of 245 healthy residents with no records of fever within 2 weeks and no history of DF infection were randomly extracted from 4 DF epidemic areas in 2014. Blood samples from residents were collected, the anti dengue virus IgG was tested by enzyme linked immunosorbent assay and was compared with samples from residents of other areas. To understand the factors of dengue infection, questionnaire investigation was done in epidemic areas, and univariate and multivariate logistic regression analysis was utilized for the data analyzing. Results In contrast with normal control subject (3. 09%, 13/421 ) ,24.49% (60/245) of the subject living in epidemic areas was seropositive for dengue virus, and the difference was significant ( x2 = 72.69, P 〈 0. 001 ). There was a negative correlation between IgG levels and age ( x2 = 7.07, P 〈 0.05 ). Via univariate and multivariate logistic regression analysis, possible factors that influence the IgG level were predicted: mosquito net usage, hydrophytes and house environment. Conchltsion The percentage of the population epidemic outbreak of dengue fever infection is high, some measures for high risk factors should be taken to prevent and control the epidemic of dengue fever.
