摘　　要：目的观察黄芪注射液对KOA的治疗效果,并从PPAR-γ表达探讨其对KOA的作用机制。方法将符合标准的60例KOA患者按照随机数字表法分为黄芪注射液组(A组)、玻璃酸钠组(B组)、氯诺昔康组(C组)三组,每组各20例。A、B、C组分别膝关节腔注射黄芪注射液、玻璃酸钠注射液和注射用氯诺昔康,每周1次,连续治疗5次。采用ISOA评估A、B组治疗前后病情,同时采用ELISA检测A、C组治疗前后膝关节液中PPAR-γ的含量。结果 A、B两组治疗前后ISOA评分比较,差异有显著性意义(P<0.05);A、B两组治疗后ISOA评分比较,差异无显著性意义(P>0.05)。A、C两组治疗前后PPAR-γ含量比较,差异有显著性意义(P<0.05);A、C两组治疗后PPAR-γ含量比较,差异无显著性意义(P>0.05)。结论黄芪注射液对KOA具有治疗作用,作用机制可能与上调PPAR-γ表达有关。Objective To investigate the therapeutic effect of Astragalus injection on Knee Osteoarthritis （KOA） and explore the mechanism of KOA by PPAR -γ expression. Methods 60 patients diagnosed with KOA were randomly di- vided into Astragalus injection group （Group A） , Sodium Hyaluronate injection group （Group B） and Lornoxicam injection group （ Group C） , 20 cases in each group. Groups A, B and C were treated with intra - articular injection of Astragalus in- jection, Sodium Hyaluronate injection and Lornoxicam injection respectively once a week for 5 consecutive weeks. The thera- peutic conditions were assessed by Index of Severity for Osteoarthritis （ISOA） and the expressions of PPAR - γ in the synovi- al fluid were measured by Enzyme Linked Immunosorbent Assay （ELISA） respectively. Results There was significant difference in the ISOA scores between groups A and B before treatment （ P 〈 0. 05 ）. But there was no significant difference in the ISOA scores between them B after treatment （ P 〉 0. 05 ）. There was significant difference in the expressions of PPAR -γ between Groups A and C before treatment （ P 〈 0. 05 ）. There was no significant difference in the expressions of PPAR - γ ei- ther between them after the treatment （ P 〉 0. 05 ）. Conclusion Astragalus injection can effectively improve KOA. The ther- apeutic mechanism of Astragalus injection on KOA may be related to the up - regulation of PPAR -γ expression.
