多囊卵巢综合征超声影像特征及与性激素、胰岛素抵抗指数的相关性分析

摘　　要：[目的]探讨多囊卵巢综合征（PCOS）患者超声影像特征与性激素、胰岛素抵抗指数（HOMA-IR）的相关性。[方法]选取2014年2月至2016年5月本院收治的PCOS患者90例（观察组），另选取同时期在本院进行健康体检者53例（对照组），对两组患者进行超声影像检查，检查指标包括卵巢总面积（TA）、卵巢间质面积（SA），同时计算SA／TA比值，检测两组患者的性激素水平及血糖等相关指标水平，并分析观察组患者的超声影像特征与性激素指标、HOMA-IR的相关性。[结果]与对照组对比，观察组患者的黄体生成素（LH）、睾酮（T）、雌二醇（E2）及空腹血糖（FBG）、HOMA-IR明显升高，其差异均有统计学意义（P〈0．05）；与对照组对比，观察组患者的TA、SA及SA／TA明显升高，差异有统计学意义（P〈0．05）；Person相关性分析显示，观察组患者的SA与LH、T、E2呈显著正相关（r=0．280、0．610、0．389，P〈0．05），SA／TA与LH、LH、T、E。也呈显著正相关（r=0．265、0．378、0．332，P〈0．05），TA与E2存在显著正相关（r=0．315，P〈0．05）。SA／TA比值与HOMA-IR呈显著正相关（r=0．712，P〈0．05）。[结论]PCOS患者卵巢SA及TA明显高于健康体检者，且超声影像特征与性激素、HOMA-IR存在显著相关性。[Objective]To investigate the correlation between ultrasonographic features and sex hormones and in- sulin resistance index in patients of polycystic ovary syndrome （PCOS）. [Methods] Ninety patients with PCOS from February 2014. to May 2016 in our hospital were selected as observation group, and 53 cases in the same period for health examination were selected as control group. Ultrasonic imaging detection was performed in the two groups of patients, the detecting indexes included ovarian total area （TA）, ovarian stromal area （SA） ;At the same time the ratio of SA/TA was calculated, sex hormone levels, blood glucose levels and other related indicators were detected in the two groups of patients , and correlation between the sonographic characteristics and sex hormone index and the HOMA-IR of patients in the observation group was analyzed. [Results] Compared with the control group, the lutein- izing hormone （LH）, testosterone （T）, estradiol （E2） and fasting blood glucose （FBG） and HOMA-IR of the ob- servation group of patients increased significantly, the differences were statistically significant （ P 〈0.05） ;compared with the control group, TA, SA and SA/TA of the observation group of patients increased significantly, the differ- ence was statistically significant （ P %0.05） ;Person correlation analysis showed that SA of the observation was posi- tively correlated with LH, T, E2 significantly（ r = 0.280,0.610,0.389, P 〈0.05）;SA/TA, and LH, LH, T,E2 also showed a significant positive correlation （ r = 0.265,0.378,0.332, P〈0.05） ;there were significant positive cor- relation between TA and E2（ r = 0.315, P〈0.05）.SA/TA ratio was positively correlated with H OMA-IR （ r = 0.712, P 〈0.05）. [Conclusion] Ovarian SA and TA of patients with PCOS are significantly higher than those of healthy physical examination, and the ultrasonic image characteristics are significantly correlated with sex hormones and HOMA-IR.
