老年着色性干皮病继发恶性肿瘤1例

摘　　要：患者男,67岁。面部、躯干及四肢皮疹60年,右眼睑肿物4个月伴渗出。皮肤科情况:面、躯干多发针尖至米粒大小褐色、黑色斑疹,丘疹,下唇及鼻部可见线性瘢痕,右下眼睑约蚕豆大小肿物,表面溃疡、脓性渗出物、结痂。肿物组织病理示:真皮内部分浸润细胞核大、深染,部分区域可见角珠形成,个别核丝分裂相,中低分化鳞状细胞癌。诊断:着色性干皮病继发鳞状细胞癌。A 67-year-old male presented with lesions on the face, trunk and distal limbs for 60 years. In recent 4 months, a neoplasm presented with exudation on the right eyelid. Clinical examination showed multiple brown or black macules and papules on the face and trunk, the size of lesions were needlepoint to granule, linear scar appeared on the lower lip and nose. A broadbean size neoplasm on the right eyelid, the surface of the neoplasm was ulcer with exudation and crusting. Histopathology of the neoplasm showed part of infiltrating cell nucleus were prominent with mitosis in the dermis, keratin pearls were seen in part of zone, poorly to moderately differentiated squamous cell carcinoma. The diagnosis was xeroderma pigmentosum with secondary squamous cell carcinoma.
