蠲痹泻肺方联合西药治疗结缔组织病早期肺动脉高压随机平行对照研究

摘　　要：[目的]观察蠲痹泻肺方联合西药治疗结缔组织病早期肺动脉高压疗效。[方法]使用随机平行对照方法,将62例住院/门诊患者按就诊顺序号方法简单随机随为两组。对照组30例硝苯地平,10mg/次,2次/d;若不耐受,改用卡托普利,25mg/次,2次/d。治疗组32例蠲痹泻肺方(黄芪、五爪龙各30g,南蛇藤20g,青风藤30g,穿山龙15g,防己10g,秦艽15g,丹参20g,莪术、桑白皮、地骨皮15g,葶苈子5g,茯苓30g,大枣、甘草各10g),1剂/d,水煎200m L,早晚温服;西药治疗同对照组。连续治疗12周为1疗程。观测临床症状、静息肺动脉收缩压(PASP)、6min步行距离(6mwd)、不良反应。治疗1疗程,判定疗效。[结果]治疗组痊愈8例,显效19例,无效5例,总有效率84.40%;对照组痊愈4例,显效12例,无效14例,总有效率53.30%;治疗组疗效优于对照组(P<0.05)。PASP、6min步行距离两组均有改善(P<0.01),治疗组改善优于对照组(P<0.05,P<0.01)。[结论]蠲痹泻肺方联合西药治疗结缔组织病早期肺动脉高压,疗效满意,无严重不良反应,值得推广。[Objective] To observe the Juanbi xiefei decoction combined with western medicine in treatment of connective tissue disease early pulmonary hypertension curative effect. [Method] Using random parallel control method, 62 cases of inpatient/outpatient patients according to the order number randomly into two groups. The control group with 30 cases of nifedipine, 10 mg/times, 2 times/d; if not by tolerance, captopril, 25 mg/times, 2 times/d. treatment group of 32 cases of Juanbi xiefei party（Huang Qi, Ipomoea dragon 30 g, Celastrus 20 g, orientvine 30 g, from 15 g 10g 15 g, tetrandrine, Gentiana, Salvia 20 g, rhizoma curcumae, morus alba, Digupi 15 g, Tinglizi 5g, Poria 30 g, jujube, licorice, 1） 10 g agent/d, Decoction 200 m L, sooner or later the day; westernmedicinetreatmentwiththecontrol group. 12 weeks of continuous treatment for 1 course of treatment. Observe the clinical symptoms, pulmonary artery systolic pressure（PASP）, 6min walking distance（6MWD）, adverse reactions. To judge the curative effect of treatment of 1 treatment. [Result] The treatment group 8 cases were cured, 19 cases markedly effective, 27 cases effective, 5 cases ineffective, the total efficiency of 84.40%; control group 4 cases were cured, 12 cases markedly effective, 16 cases effective, 14 cases ineffective, the total efficiency of 53.30%; the treatment group than the control group（P〈0.05）. PASP, 6min walking from the two groups were improved（P〈0.01）, the treatment group was better than the control group（P〈0.05, P〈0.01）. [Conclusion] Juanbi xiefei decoction combined with western medicine in treatment of connective tissue disease early pulmonary hypertension, the curative effect is satisfied, no serious adverse reactions, worthy of promotion.
