摘 要:目的:探讨维持性血液透析(MHD)患者脂肪因子与血管钙化的相关性。方法:选择2013年9月起笔者所在医院肾内科CKD5期患者90例,其中MHD患者60例为MHD组,非透析患者30例为对照组。比较两组患者临床资料、糖脂代谢、炎症因子、脂肪因子及冠脉钙化积分。MHD组冠脉钙化积分与脂肪细胞因子、血脂、炎症因子进行相关分析。结果:MHD组与对照组相比,在年龄、性别无显著差异情况下,ADPN、LEP、TG、CRP、A/L比值、冠脉钙化积分均高于对照组(P<0.01),HDL明显低于对照组(P<0.01),Alb与对照组相比差异有统计学意义(P<0.05)。MHD组冠脉钙化积分与HDL、ADPN、A/L比值呈负相关(P=0.021、0.024、0.000),与CRP、LEP呈正相关(P=0.001、0.025),与年龄、BMI、TG、TC、LDL、Alb、FSG的相关性不显著(P>0.05)。结论:MHD患者脂联素、瘦素明显升高,可能参与了糖脂代谢紊乱、营养不良、微炎症的发生,直接或间接促进血管钙化的发生发展。衡量血管钙化,A/L比值敏感性可能更强。Objective: To investigate the correlation between adipokines and vascular calcification in maintenance hemodialysis(MHD) patients. Method: 90 cases of CKD5 patients in our hospital were included from September 2013.Including 60 cases of maintenance hemodialysis(MHD) patients as MHD group, 30 cases of non dialysis patients as control group.The clinical data, blood glucose and lipid metabolism, inflammatory factors, adipokines and coronary artery calcification score were compared between the two groups.MHD group of coronary artery calcification score and the fat cell factors, blood lipid, inflammatory factors were analyzed.Result: Compared with the control group, the MHD group had no significant difference in age and sex, the Serum ADPN, LEP, TG, CRP, A/L ratio and coronary artery calcification score of the MHD group were higher than those of the control group(P〈0.01).HDL significantly lower than the control group(P〈0.01), Alb of the two groups also had significantly statistical difference(P〈0.05).In the MHD group, the coronary artery calcification score was negatively correlated with HDL, ADPN, and A/L ratio(P=0.021, 0.024, 0.000), which was positively correlated with CRP and LEP(P=0.001, 0.025).The correlation with age, BMI, TG, TC, LDL, Alb, FSG was not significant(P〉0.05).Conclusion: Adiponectin and leptin are increase in MHD patients.They may participate in the occurrence and development of glucose and lipid metabolism disorders, malnutrition, micro inflammation, directly or indirectly promote the occurrence and development of vascular calcification.Measure of vascular calcification, A/L ratio may be more sensitive.
