低剂量舒芬太尼复合左布比卡因腰麻在剖宫产中的效果评价

摘　　要：目的:探究低剂量舒芬太尼复合左布比卡因腰麻在剖宫产中的效果。方法:随机选取2014年8月-2016年2月在笔者所在医院进行剖宫产的产妇100例,对其相关临床资料进行回顾性分析,将腰麻用药为0.5%左布比卡因10 mg的30例产妇作为A组;将在A组基础上加用2.5μg舒芬太尼的40例作为B组,将在A组基础上加用5μg舒芬太尼的30例作为C组,观察对比三组的麻醉效果。结果:B组和C组T10镇痛维持时间明显长于A组,肌松情况评分高于A组(P<0.05);与A组和B组相比,C组出现低血压的例数以及麻黄碱用量明显增加(P<0.05),B组和C组寒战的发生率明显低于A组(P<0.05)。结论:采用2.5μg和5μg舒芬太尼和0.5%左布比卡因复合腰麻均具有良好的麻醉效果,保证手术效果,并延长了患者术后的镇痛时间,但是采用2.5μg舒芬太尼进行复合麻醉,出现低血压的例数明显减少。Objective： To explore the effect of Sufentanil combined with low dose of Levobupivacaine in spinal anesthesia of cesarean section. Method： 100 cases of cesarean section puerpera in our hospital from August 2014 to February 2016 were randomly selected, the relevant clinical data were retrospectively analyzed.The bslomg 30 cases of maternal 0.5% Levobupivacaine 10 mg were as group A, 40 cases with 2.5 μg Sufentanil on the basis of group A were as group B, 30 cases with 5μ g Sufentanil on the basis of group A were as group C.The anesthesia effect of the three groups were compared.Result： The Tl0 analgesia time of group B and group C were longer than group A, the muscle relaxation scores were higher than group A（P〈0.05）.Group C compared with group A and group B, the number of patients with hypotension and dose of ephedrine were significantly increased（P〈0.05）.The incidence rates of chill in group C and group B were lower than group A（P〈0.05）.Conclusion： Using 2.5 μ g and 5 μg Sufentanil and 0.5% Levobupivacain in combined spinal anesthesia has good anesthetic effect, it can ensure the operation effect and prolong the time of postoperative analgesia, but with 2.5μg Sufentanil for anesthesia, the number of cases of hypotension significantly reduced.
