甘油果糖联合呋塞米治疗高血压脑出血合并急性肾损伤及心功能不全患者的疗效研究

摘　　要：目的:评价甘油果糖联合呋塞米治疗脑出血患者合并急性肾损伤及心功能不全的疗效及安全性。方法:选取湖北医药学院附属人民医院自2012年9月至2016年4月期间收治的高血压脑出血,且同时合并急性肾损伤及心功能不全的患者36例。随机分为治疗组及对照组,每组18例。治疗组给予甘油果糖和呋塞米联合脱水,对照组给予20%甘露醇和呋塞米联合脱水。2周后对肾功能、心功能恢复情况进行比较;1月后对两组患者进行疗效分析和临床神经功能缺损程度评分。结果:治疗组的总有效率优于对照组;治疗组临床神经功能缺损评分方面较对照组改善明显,但均无统计学意义(P>0.05)。治疗组患者肾功能及心功能改善情况均优于对照组,两组差别具有统计学意义(P<0.05)。治疗组较对照组不良反应少,安全性好。结论:甘油果糖联合呋塞米治疗高血压脑出血患者合并急性肾损伤及心功能不全的疗效肯定,且安全性较高。Objective To assess the effects and safety of glycerine fructose combined furosemide on treatment of hypertension cerebral cemorrhage patients complicated acute kidney injury and cardiac insufficiency. Methods From September 2012 to April 2016,36 hypertension cerebral hemorrhage patients complicated acute kidney injury and cardiac insufficiency were enrolled in this study in our hospital.Then they were randomly divided into treatment group and control group（ n = 18）.The patients were respectively treated with glycerine fructose combined furosemide in treatment group and 20% mannitol combined furosemide in control group.The recovery situation of renal and heart function were compared after two weeks,the curative effects and the score of clinical neurologic impairment were analyzed after one month. Results The total effective rate of treatment group was superior to that of control group,the improvement of clinical neurologic impairment score in treatment group was more obvious than that in control group,but the difference had no significance（ P〉0.05）.The recovery situation of renal and heart function in treatment group were significantly superior to that in control group（ P〈0.05）.The safety of treatment group was better than that of control group with its less adverse reactions.Conclusion Glycerine fructose combined furosemide could be effectively and safety used for treating hypertension cerebral hemorrhage patients complicated acute kidney injury and cardiac insufficiency.
