摘  要:目的:观察自制氧驱动雾化吸入回路对全麻气管导管拔管期不良反应的影响。方法:将60例拟行全麻气管插管手术患者,按随机数字表法均分为试验组和对照组。试验组气管导管拔管期采用氧驱动雾化吸入(2%盐酸利多卡因5 m L+地塞米松5 mg+生理盐水10 m L),对照组采用常规方法拔管。记录两组患者的耐管评分、拔管时间,记录围拔管期并发症如拔管前、后血流动力学改变,咽部不适,咽喉痛和声音嘶哑发生率。结果:试验组耐管评分明显低于对照组、拔管时间明显长于对照组(P<0.05),试验组围拔管期血流动力学平稳,咽部不适、咽喉痛和声音嘶哑发生率明显低于对照组(均P<0.05)。结论:自制氧驱动雾化吸入回路用于全麻气管导管拔管期管理具有一定临床应用价值。Objective: To study the clinical efficacy of self-made oxygen-driven inhalation loop phase in general anesthesia tracheal extubation period. Methods: A total of 60 patients undergoing operation with tracheal intubation general anesthesia were randomly divided into two groups, 30 cases in each group. Selfmade oxygen driven inhalation was used in experimental group ,while no oxygen-driven inhalation was used in control group. The resistance tube rating and extubation time were recorded in two groups. Extubation complications, such as change of hemodynamics, throat discomfort, sore throat and hoarseness were recorded in all patients. Results: There were no difference in age, sex and weight between two groups(P 〉 0.05 ). The resistance tube rating and extubation time in experimental group was significantly longer than that in control group ( P 〈 0.05 ). Compared with control group, hemodynamic changes were tiny and the complications of extubation such as throat discomfort, sore throat and hoarseness were obviously fewer in experimental group ( P 〈 0.05 ). Conclusion: The self-made oxygen driven inhalation anesthesia circuit of tracheal extubation period is a relatively safe, effective method, and is worthy of clinical application.
