自拟补肾调肝止颤汤治疗肝肾不足型帕金森病临床研究

摘　　要：目的:观察自拟补肾调肝止颤汤治疗肝肾不足型帕金森病(PD)的临床疗效。方法:选取70例PD患者随机分为观察组和对照组各35例。对照组采用美多芭配合安慰剂治疗,观察组则使用美多芭联合自拟补肾调肝止颤汤内服,观察2组治疗中左旋多巴的日均摄入量、治疗后的疗效及治疗前后PD评定量表(UPDRS)、PD睡眠量表(PDSS)、汉密尔顿抑郁量表(HAMD)、简短精神状态量表(MMSE)、PD生活质量量表(PDQ-39)、中医证候积分的改善情况,记录2组不良反应发生情况。结果:治疗1个疗程后,2组各项评分均有显著改善,但观察组改善幅度更大(P<0.05);而1个疗程结束后6个月,观察组各项指标均较疗程结束时变化不大,但对照组却发生明显变化(P<0.05),而与同期观察组比较,其UPDRSⅠ-Ⅳ、HAMD、PDQ-39及中医证候评分均明显较高而PDSS、MMSE明显较低(P<0.05)。观察组平均每日左旋多巴的摄入量显著少于对照组,而有效率85.7%则显著高于对照组57.1%(P均<0.05);但2组不良反应发生率差异无统计学意义。结论:补肾调肝止颤汤联合美多芭能够改善肝肾不足型PD患者运动症状和非运动症状,提高患者生活质量,减轻药物的不良反应,值得临床推广。Objective：To observe the clinical effect Self-made Bushen Tiaogan Zhichan decoction in treating Parkinson＇ s disease（ PD）.Methods：Total 70 PD patients were randomly divided into the control group（ 35 cases） and observation group（ 35 cases）.The control group received madopar with placebo.The observation group received madopar added Bushen Tiaogan Zhichan decoction.The clinical effect after the treatment,dosage of madopar per day in treatment and scores of unified PD rating scale（ UPDRS）,Parkinson＇s disease sleep scale（ PDSS）,Hamilton Depression scale（ HAMD）,Mini-Mental State Examination（ MMSE）,PD quality of life scale（ PDQ-39）,Chinese medicine syndrome scores,as well as the adverse effect were recorded before and after the treatment.Results：After one treatment course,the scores of the two groups improved,but the observation group had more obvious change（ P〈0.05）;six months after the treatment,the scores of the observation group changed little,but the the control group changed greatly（ P〈0.05）.when comparing the two groups at the same time period,the results of UPDRSⅠ-Ⅳ score,HAMD,PDQ-39,Chinese medicine syndrome scores in the observation group were lower than those in the control group（ P〈0.05）.the score of MMSE and PDSS in observation group were higher than control group（ P〈0.05）.The dosage of madopar in the observation group was less than the control group（ P〈0.05）;the total effective rate of the observation group was 85.7%,significantly higher than control group＇s 57.1%（ P〈0.05）;the adverse reaction in the two group was not clear.Conclusion：Selfmade Bushen Tiaogan Zhichan decoction can relieve motor and non-motor symptoms and improve patients＇ quality of life,reduce the adverse effect,which is worthy of clinical promotion.
