摘　　要：目的:探讨加味大承气汤联合西药对急性胰腺炎患者疗效、微炎性反应状态、血清淀粉酶及血清钙的影响。方法:急性胰腺炎患者98例按照随机数字表法随机分为观察组(n=49)与对照组(n=49)。观察组采用加味大承气汤联合西药治疗,对照组仅采用常规西药治疗。2组疗程均为10 d。比较2组患者治疗疗效,胃肠道功能恢复时间、WBC恢复时间和住院时间,治疗前后血浆IL-6、IL-8、TNF-α水平及血淀粉酶和血钙水平变化,及不良反应发生情况。结果:观察组总有效率(91.84%)高于对照组(75.51%)(P<0.05);观察组胃肠道功能恢复时间、WBC恢复时间和住院时间优于对照组(P<0.05);观察组治疗后血浆IL-6、IL-8、TNF-α水平降低(P<0.05);观察组治疗后血浆IL-6、IL-8、TNF-α水平低于对照组(P<0.05);2组治疗后血淀粉酶增加,而血钙降低(P<0.05);观察组治疗后血淀粉酶高于对照组,血钙低于对照组(P<0.05);在用药期间均未出现严重不良反应。结论:加味大承气汤联合西药治疗急性胰腺炎疗效显著,可明显改善患者微炎性反应状态,降低血钙水平,增加血淀粉酶水平,具有重要研究价值。Objective：To investigate the effect of modified Da Cheng Qi Decoction Combined with western medicine on curative effect,micro inflammation state,serum amylase and serum calcium in patients with acute pancreatitis .Methods：A total of 98 cases of acute pancreatitis were randomly divided into observation group （n=49）and control group（n=49）according to the random num-ber table method.The observation group was treated with modified Da Cheng Qi Decoction Combined with western medicine ,and the control group was treated with routine western medicine only .The course of the two groups were all 10 d.Treatment,the curative effect is compared between the two groups of patients ,gastrointestinal function recovery time ,WBC recovery time and hospitalization time,before and after the treatment ,the plasma levels of IL-6,IL-8 and TNF-αlevel and serum amylase and calcium levels ,and ad-verse reactions .Results：The total effective rate of the observation group （ 91.84%） was higher than that of the control group （75.51%）（P〈0.05）;The gastrointestinal function recovery time ,WBC recovery time and hospitalization time in the observation group were better than those in the control group （P〈0.05）;After treatment,the plasma levels of IL-6,IL-8 and TNF-αwere de-creased in the observation group （P〈0.05）;The level of IL-6,IL-8 and TNF-αin the observation group after treatment was lower than that in the control group （ P〈0.05）;The two groups after treatment of blood amylase increased ,and decreased blood calcium （ P〈0.05）;The observation group after treatment ,serum amylase was higher than the control group ,serum calcium was lower than that of control group （ P〈0.05）;No serious adverse reactions occurred during the administration period .Conclusion：Modified Da Cheng Qi Decoction Combined with western medicine for the treatment of acute pancreatitis has a significant effect ,can significantly improve the micro inflammatory state in patients ,reduce blood calcium levels ,increase
