高位结肠透析联合中药保留灌肠对慢性肾衰患者肾功能的影响

摘　　要：目的:观察高位结肠透析联合中药保留灌肠治疗慢性肾衰(Chronic Renal Failure,CRF)的疗效。方法:使用常规治疗32例患者(A组),与常规治疗+中药保留灌肠31例患者(B组),以及常规治疗+高位结肠透析联合中药保留灌肠治疗34例患者(C组)进行随机对照,观察每组治疗前后尿素氮(Urea Nitrogen,Bun)、血清肌酐(Serum Creatinine,Scr)、血清尿酸(Uric Acid,UA)、内生肌酐清除率(Creatinine Clearance Rate,Ccr)、胱抑素C(Cystatin C,Cys C)、甲状旁腺激素(Parathyroid Hormone,PTH)变化情况,比较3组疗效。结果:C组治疗后在降低Bun、Scr、UA、Cys C、PTH,升高Ccr方面与治疗前有着统计学意义(P<0.01),B组治疗前后同样具有统计学意义(P<0.05),同时C组较B组占优(P<0.05);而A组治疗前后改善不理想(P>0.05)。结论:提示高位结肠透析联合中药保留灌肠能明显改善肾功能,降低PTH,延缓CRF的进展,且优于传统中药灌肠以及常规治疗。Objective：To observe the effect of high colon dialysis combined with Chinese medicine retentive enema treatment for chronic renal failure （ CRF） .Methods：Thirty-two patients received conventional treatment （ Group A） and 31 patients were given conventional treatment combined with Chinese medicine retentive enema （ Group B） and 34 cases were treated by conventional treatment plus high colon dialysis combined with traditional Chinese medicine retentive enema （Group C）.The change of urea ni-trogen （Bun）, serum creatinine （Scr）, serum uric acid （UA）, endogenous creatinine clearance urinary inhibition （Ccr）, Cysta-tin C （ CysC） and parathyroid hormone （ PTH） were observed before and after in each group .Results：Group C showed significant difference after treatment in reducing Bun , Scr, UA, CysC, PTH and rising Ccr （P〈0.01）.Group B also showed the difference before and after the treatment （P〈0.05）, while the effect of Group C was better than that of the Group B （P〈0.05）.The im-provement in Group A was not good after treatment （P〉0.05）.Conclusion：High colon dialysis combined with Chinese medicine retain enema may significantly improve renal function , reduce the PTH , delay the progress of CRF , and is superior to the combina-tion of traditional Chinese medicine enema and conventional treatment .
