益肺健脾颗粒改善老年慢阻肺患者肺功能及免疫功能

摘　　要：目的:探讨益肺健脾颗粒对老年慢阻肺患者(肺脾气虚型)的肺功能及免疫功能的影响。方法:选取2013年6月至2015年6月新疆医科大学附属中医医院VIP病房110例COPD老年患者,将其平分为观察组和对照组,每组55例,其中对照组使用临床常规治疗,观察组在此基础上加用益肺健脾颗粒治疗,3个疗程后观察比较2组患者的肺功能、免疫功能、中医证候积分及疗效。结果:2组患者治疗前PEF、FEV1、FEV1/FVC等肺功能指标比较,差异无统计学意义(P>0.05)。经相应治疗后,2组患者肺功能均有所改善,但观察组治疗后各指标明显比对照组要高(P<0.05),差异有统计学意义。2组治疗前CD3+、CD4+/CD8+、Ig A、Ig M检测值差异无统计学意义(P>0.05),治疗后观察组CD3+、Ig A、Ig M水平明显升高,CD4+/CD8+比值也明显增加,与对照组治疗后比较,差异有统计学意义(P<0.05)。治疗后,观察组的证候积分明显比对照组降低,且观察组的总有效率为89.09%,也明显比对照组的65.45%高,2组比较差异有统计学意义(P<0.05)。结论:益肺健脾颗粒能有效改善COPD稳定期肺脾气虚型患者的肺功能及临床症状,增强患者免疫功能,提高患者临床疗效,值得在临床上使用。Objective：To study the effect of Yifei Jianpi Granule on lung function and immune function in elderly patients with COPD（feipiqixu）.Methods：Selected 110 cases elderly patients with COPD to average divided in control group and observation group.The control group used routine treatment and observation group used Yifei Jianpi Granule on the basis of control group.Observed and compared the lung function、immune function、TCMsyndrome scoring and effect in the two groups after 3 courses.Results：The PEF、FEV1 、FEV1 /FVC was no significant difference between two groups before treatment（P 〉0.05）and they had improved after treatment.The lung function after treatment in observation group was better than that in control group.It had significant difference between two groups（P 〈0.05）.The CD3 ^＋、CD4 ^＋/CD8 ^＋、IgA、IgMwas no significant difference between two groups before treatment （P 〉0.05）.The CD3 ^＋、IgA、IgM、CD4 ^＋/CD8 ^＋ increased in observation group after treatment.The immune function after treatment in observation group was better than that in control group（P 〈0.05）.It had significant difference.The TCM syn-drome scoring in observation group was lower than that in control group（P 〈0.05）and the total effective rate of observation group was higher than that in control group（89.09% VS 65.45%）（P 〈0.05）.The difference was statistically significant.Conclusion：Yifei Jianpi Granule can resolve clinical symptoms and improve lung function,immune function and the effect in elderly patients with COPD（feipiqixu）.It is valuable to advocate in clinic.
