摘　　要：该文阐述高能量胫骨干开放性骨折的诊断治疗的最新进展。回顾近20年来的文献,分析胫骨干开放性骨折的分型、治疗方法的特点,归纳各种治疗方式的优缺点。处理高能量胫骨开放性骨折的同时,必须重视对小腿软组织的修复,才能减少骨髓炎的发生,促进骨折的愈合。清创后重新评估软组织的损伤程度,对选择合适的手术方式、手术材料以及骨折的预后至关重要。早期处理开放性骨折、合适的固定方式及良好的软组织覆盖是高能量胫骨干开放性骨折的冶疗关键。The paper explored latest progress in the treatment of open tibial fracture with high energy injury. The characteristics of the classification and treatment of open tibial shaft fracture was analyzed, and the advantages and disadvantages of various treatment methods was summarized according to the literatures reviewed in the past 20 years. It should pay more attention to the repair of soft tissue injury when treating the high energy tibial open fracture at the same time, in order to reduce the occurrence of osteomyelitis, promote the healing of fracture. The reassessment of injury severity of soft tissue was of great importance for choosing suitable surgical method, surgical materials and prognosis of fractures. Early treatment of open fractures, appropriate fixation, and good soft tissue coverage is the key for the treatment of high energy open tibial shaft fracture.
