保留胫骨止点半腱肌重建兔前交叉韧带腱骨愈合实验分析

摘　　要：目的观察保留胫骨止点半腱肌重建兔前交叉韧带术后1年内腱骨愈合过程。方法取新西兰大白兔半腱肌并保留胫骨止点重建前交叉韧带,于术后2周、4周、8周、3月、6月、12月分别采取标本。采用HE和甲苯胺蓝染色观察骨隧道内腱骨愈合的过程。结果术后2周移植的肌腱与骨隧道壁连接紧密可见大量的成纤维细胞,胶原排列不整齐。术后4周移植的肌腱与骨隧道壁的腱骨间形成纤维连接并有少量纤维软骨形成。术后8周移植的肌腱与骨隧道壁的腱骨间的组织出现分层,隐约可见4层结构并隐约可见潮线。术后3个月形成类似直接止点的4层结构,潮线较前明显。术后6个月形成直接止点的4层结构,潮线较前更明显。术后12个月形成了成熟的直接止点的结构,潮线清晰可见。结论保留胫骨止点兔自体半腱肌重建前交叉韧带骨隧道内腱骨愈合是由间接的固定点逐渐转化为直接止点的过程。Objective To observe the healing process of using semitendinosus and keeping the tibial insertion to reconstruct rabbit anterior cruciate ligament in 1 year. Methods Taking specimens respectively of New Zealand white rabbit,which using semitendinosus and keeping the tibial insertion to reconstruct anterior cruciate ligament,in 2weeks,4 weeks,8 weeks,3 months,6months and 12 months after the operation. To observe tendon-bone healing process in the bone tunnel by HE and toluidine blue staining. Results 2 weeks after surgery we observed well connection of transplanted tendon and bone tunnel,and large numbers of fibroblasts,but the arrangement of collagen was not neat. After 4 weeks we also found the tendon-bone fiber connection of transplanted tendon and bone tunnel,with formation of a small amount of fibrous cartilage. 8 weeks after the transplantation,the tendon-bone tissue between tendon and bone tunnel wall appeared four layer structure and tide line. 3 months after the operation,we observed the four layers of structure forming which was similar to tibial insertion,and more significant tide line. 6 months after the operation we found form directly tibial insertion＇s four layer structure,tide line more clearly than before. We observed the structure of the mature directly tibial insertion,and the tide line was clearly visible after 12 months. Conclusion The the process of tendon-bone healing by using semitendinosus and keeping the tibial insertion to reconstruct rabbit anterior cruciate ligament is the process of turning indirect point gradually into direct point.
