摘 要:目的:分析研讨经肱动脉行介入治疗下肢动脉硬化闭塞症的临床效果。方法:此项为前瞻性研究,采用随机抽签法抽取2014年2月-2015年8月本院收治的96例下肢动脉硬化闭塞症患者,按入院顺序分为对照组和研究组,各48例。对照组采用经股动脉入路介入治疗,研究组采用经肱动脉介入治疗,对比两组的TTS、ABI、并发症。结果:研究组一次性穿刺成功率为81.25%,高于对照组的64.58%,差异有统计学意义(P<0.05)。两组的手术成功率、随访中复发率、随访中死亡率比较,差异均无统计学意义(P>0.05)。术前和随访6个月,两组的ABI、TTS比较,差异均无统计学意义(P>0.05)。研究组并发症总发生率为4.17%,低于对照组的16.67%,差异有统计学意义(P<0.05)。结论:临床治疗下肢动脉硬化闭塞症疾病可考虑采用经肱动脉行介入,可按照患者状况确定手术入路方式,疗效突出,且降低了并发症发生率。Objective:To study and analyze the clinical effect by brachial artery interventional treatment of lower extremity arterial occlusive disease.Method:This study was a prospective study,and case selection method was random ballot,and 96 cases with lower extremity arterial occlusive disease in our hospital from February 2014 to August 2015 were included in the discussion according to the method of random sampling,they were divided into the control group and the study group according to the order of admission,48 cases in each group.The control group was treated with interventional therapy via femoral artery approach,the study group was treated with interventional therapy via brachial artery.The ABI,TTS and complications were compared between the two groups.Result:The disposable puncture success rate of study group was 81.25%,higher than 64.58% in control group,the difference was statistically significant(P〉0.05).The success rate of surgery,follow-up recurrence rate,comparing mortality during follow-up of two groups,there were no significant differences(P〉0.05).Before the operation and at six months of follow-up,the ABI and TTS of the two groups were compared,the differences were no statistically significant(P〉0.05).The overall complication rate of study group was 4.17%,lower than 16.67% in control group,the difference was statistically significant(P〈0.05).Conclusion:Clinical treatment of lower extremity arterial occlusive disease may be considered via brachial artery intervention,according to the patients condition to determine the surgical approach,prominent curative effect,and reduce the incidence of complications.
