微创经皮钢板螺钉内固定治疗胫骨远端骨折的疗效观察

摘　　要：目的探讨应用微创经皮钢板螺钉内固定术(MIPPO)治疗胫骨远端骨折的疗效。方法回顾2013年1月—2014年12月应用MIPPO技术治疗的28例胫骨远端骨折患者,术后1、2、3、6、12、24个月定期复查X线片并行临床功能检查,记录骨折愈合时间及有无并发症,按照Johner-wruhs胫骨骨折疗效评定标准进行功能评价。结果所有28例患者均获得随访,随访时间10~24个月,平均(14.3±4.3)个月。术后均获得骨愈合,骨愈合时间12~24周,平均(15.1±3.2)周。术后无伤口感染、骨折延迟愈合、不愈合等并发症发生。按照Johnerwruhs胫骨骨折疗效评定标准进行疗效评定,优良率92.9%。结论应用MIPPO技术治疗胫骨远端骨折创伤小,功能恢复佳,并发症少,疗效可靠。Objective To investigate the effect of minimally invasive percutaneous plate osteosynthesis （MIPPO） in the treatment of distal tibial fractures. Methods 28 patients with distal tibial fracture were treated with MIPPO from January 2013 to December 2014. The clinical and radiological examinations were done at 1, 2, 3, 6, 12, and 24 months after operation. The fracture healing time and complications were recorded. The clinical functions were evaluated according to the Johner-wruhs standard of tibial fractures. Results 28 patients were followed up for a period of 10 to 24 months with an average of （14.3：t：4.3） months. The bone healing time was 12 to 24 weeks after operation, with an average of （15.1：t：3.2） weeks. There were no complications such as wound infection, delayed union and nonunion. According to the curative effect evaluation standard of Johner- wruhs tibial fracture, the excellent and good rate was 92.9%. Conclusion MIPPO for distal tibial fractures has advantagesof less invasion, fewer complications and satisfactory ankle function.
