小剂量布比卡因联合芬太尼坐位腰麻用于肛周手术的临床效果观察

摘　　要：目的:探讨小剂量布比卡因联合芬太尼坐位腰麻用于肛周手术的临床效果。方法:挑选2014年7月-2016年7月笔者所在医院收治的64例肛周手术患者随机分成观察组与对照组,每组32例。两组经腰椎穿刺后,观察组注入0.5%布比卡因2.5 mg(0.5 ml)、芬太尼25μg(0.5 ml),对照组注入0.5%布比卡因5 mg(1 ml),注药结束后,患者静坐10 min,当麻醉平面到达S3时执行肛周手术。对比两组麻醉效果、麻醉满意度及术中、术后并发症发生状况。结果:观察组切口感觉恢复时间、自主排尿时间、下肢运动恢复时间均低于对照组,差异有统计学意义(P<0.05);观察组麻醉满意度高于对照组,差异有统计学意义(P<0.05);观察组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:小剂量布比卡因联合芬太尼,执行坐位腰麻可较好吻合肛周手术标准,麻醉效果良好,手术后没有尿潴留出现,值得推广。Objective： To investigate the clinical effect of small dose Bupivacaine combined with Fentanyl in spinal anesthesia of perianal surgery. Method： 64 cases of anal surgery patients admitted to the author＇s hospital from July 2014 to July 2016 were selected, and randomly divided into observation group and control group, with 32 cases in each group.After lumbar puncturing for two groups, the observation group was given 0.5% Bupivacaine with 2.5 mg（0.5 ml）, and Fentanyl with 25 g（0.5 ml）, the control group was given 0.5% Bupivacaine with 5rag（1 ml）, after injection, 10 min for sitting, the operation was given when the anesthesia plane arrived in S3.The effects of two groups of anesthesia, anesthesia satisfaction, and the Intraoperative and postoperative complications were compared.Result： The incision recovery time, automatic micturition time, lower limb movement recovery time of observation group were lower than control group, the differences were statistically significant（P〈0.05）.The anesthesia satisfaction of observation group was higher than control group, the difference was statistically significant（P〈0.05）.The complications of observation group was lower than control group, the difference was statistically significant（P〈0.05）. Conclusion： Small dose Bupivacaine combined with Fentanyl, spinal anesthesia can be well performed sitting perianal operation standard, good anesthetic effect, postoperative urinary retention did not appear, worthy of promotion.
