摘　　要：目的对照研究结核性与化脓性脑膜炎患者血浆及脑脊液生化指标检测结果,为两类脑膜炎的鉴别诊断提供参考。方法收集2014-2015年医院诊治的化脓性脑膜炎(化脓性脑膜炎组)与结核性脑膜炎(结核性脑膜炎组)患者临床资料各50例,患者入院后均行血浆及脑脊液中氯化物、蛋白、糖、腺苷脱氢酶、乳酸检测,比较两组患者临床指标。结果化脓性脑膜炎组血浆及脑脊液氯化物、乳酸浓度高于结核性脑膜炎组,腺苷脱氢酶低于结核性脑膜炎组,血浆蛋白浓度高于结核性脑膜炎组,两组比较差异均有统计学意义(P<0.05);化脓性脑膜炎组脑脊液与血浆氯化物比值、乳酸比值高于结核性脑膜炎组,蛋白比值、腺苷脱氢酶比值低于结核性脑膜炎组,两组比较差异有统计学意义(P<0.05)。结论结核性与化脓性脑膜炎患者血浆及脑脊液中氯化物、蛋白、腺苷脱氢酶、乳酸浓度及比值存在一定差异,对两种脑膜炎的鉴别诊断提供临床价值。OBJECTIVE To controlled study biochemical indicators of plasma and CSF in patients with tuberculous meningitis and purulent meningitis, so as to provide a reference for the differential diagnosis of two types of men- ingitis. METHODS The clinical data of patients with purulent meningitis （purulent meningitis group） and tubercu- lous meningitis （tuberculous meningitis group） from Jan. 2014 to Dec. 2015 were collected, with 50 cases in each group. The chloride, protein, sugar, adenosine dehydrogenase, and lactic acid content of plasma and CSF were detected after admission, and the results were analyzed. RESULTS The chloride, lactic acid and plasma protein content of plasma and CSF of purulent meningitis group were higher than those of tuberculous meningitis group, adenosine dehydrogenase was lower than that of tuberculous meningitis group （P〈0.05）. The chloride ratio and lactic acid ratio of CSF and plasma of purulent meningitis group were higher than those of tuberculous meningitis group, the protein ratio and adenosine dehydrogenase ratio were lower than those of tuberculous meningitis group （P〈0.05）. CONCLUSION Chloride, protein, adenosine dehydrogenase, lactate content and ratios of plasma and CSF in patients with tuberculous meningitis and purulent meningitis exist certain differences, which can provide clinical values for differential diagnosis of the two kinds of meningitis.
