摘 要:目的探讨HBV感染型原发性肝癌患者肿瘤微环境中CD_4^+、CD_8^+T细胞及血管内皮生长因子(VEGF)水平及临床意义,为临床治疗提供参考依据。方法前瞻性收集2014年1月-2016年1月收治的HBV感染型原发性肝癌患者57例为研究组,同期收集43例非HBV感染型原发性肝癌患者为对照组;比较两组患者临床指标。结果与对照组比较,研究组患者肿瘤组织微环境中CD_4^+T细胞显著增高、CD_8^+T细胞显著降低;调节性T细胞、VEGF阳性率、肿瘤组织中微血管密度值显著增加,差异均有统计学意义(P<0.05);Pearson线性相关性分析显示,HBV DNA病毒载量与CD_4^+T细胞和调节性T细胞呈正比(P<0.05);与VEGF阴性的患者相比,VEGF阳性HBV感染型原发性肝癌患者微血管密度值计数显著增加。结论 HBV感染型原发性肝癌患者调节性T细胞高表达,可能与HBV增殖有关,且VEGF阳性率增高,与肿瘤血管生成有关。OBJECTIVE To investigate the expression and clinical significances of CD4 + , CD8 + T cell and vascular endothelial growth factor (VEGF) in the tumor microenvironment in HBV infected patients with primary hepato- cellular carcinoma, so as to provide references for clinical treatment. METHODS From Jan. 2014 to Jan. 2016, 57 cases of HBV infected patients with primary hepatocellular carcinoma admitted to our hospital were collected as study group, while 43 cases of non-HBV infected primary hepatocellular carcinoma patients were collected as con- trol group. The clinical indexes of the two groups were compared. RESULTS When compared with control group, CD4 +T cells and CD8 +T cells in the tumor microenvironment in patients of study group were significantly lower, Regulatory T cells, the positive rate of VEGF, and count of mirovessel density significantly increased (P〈0.05). Pearson linear correlation analysis showed that HBV DNA load was positively associated with CD4 + T cells and regulatory T cells (P〈0.05). Compared with VEGF negative patients, the microvessel density of VEGF positive patients with HBV infected primary hepatocellular carcinoma was significantly increased. CONCLUSION HBV in- fected patients with primary hepatocellular carcinoma get a higher expression of regulatory T cells, it may be relat- ed to the proliferation of HBV. Moreover, the positive rate of VEGF is increased, which is related to the forma- tion of tumor angiogenesis.
