体外碎石与手术取石对术后尿路感染发生的影响研究

摘　　要：目的探讨上尿路结石体外碎石与手术取石对术后尿路感染的影响,为临床治疗提供参考依据。方法选择医院2013年1-12月因上尿路结石就诊的成年患者60例;按患者治疗方式的不同分为观察组(体外碎石)30例,对照组(手术取石)30例;比较两组患者病因构成、术后的尿常规检查情况、尿液中细菌含量的差异。结果观察组尿常规三项的阳性率分别为10.0%、13.3%、16.7%显著低于对照组,差异均有统计学意义(P<0.05);观察组尿液细菌阳性率为16.7%,显著低于对照组43.3%,差异有统计学意义(P<0.05)。结论上尿路结石的体外碎石治疗与手术取石治疗相比,能够显著降低术后尿路感染的风险,为临床治疗工作提供参考。OBJECTIVE To investigate the effects of extracorporeal lithotripsy and stone surgery on postoperative u- rinary tract infection of upper urinary tract calculi, so as to provide evidence for clinical treatment. METHODS A total of 60 cases of adult patients with upper urinary tract calculi in our hospital from Jan. 2013 to Dee. 2013 were selected, and were divided into observation group （ESWL group） and control group （stone surgery group）, with 30 cases in each group. The etiology, postoperative urine routine examination, and the difference in bacterial con- tent of urine of the two groups of patients were compared. RESULTS The positive rates of three routine urine test of observation group were 10 %, 13.3 %, and 16.7 %, which were significantly lower than those of control group （P〈0.05）. The positive rate of bacteria in urine of observation group was 16.7%, which was significantly lower than 43.3% of control group （P〈0. 05）. CONCLUSION Compared with stone surgery, extracorporeal lithotripsy of upper urinary tract calculi can significantly reduce the risk of postoperative urinary tract infection, providing references for clinical treatment.
