摘  要:目的探讨右冠状动脉左室瘘的病理解剖特征及超声心动图检查的诊断价值。方法分析2015年5月14日青岛市市立医院超声科诊断的1例右冠状动脉左室瘘患者的临床资料,并复习文献报告的31例患者,对右冠状动脉左室瘘病理解剖特征及超声心动图检查的声像图特点进行回顾性分析。结果本例及文献31例患者均经手术或造影证实,其中手术29例。术前超声确诊病例29例,诊断符合率90.63%;术前超声诊断不明确,结合造影确诊2例;另有1例误诊为乏氏窦瘤破裂,假阳性率为3.12%。结论超声心动图检查可以实时动态地显示右冠状动脉左室瘘的病理解剖特征及其血流动力学改变,具有无创、快捷、高效、准确等特点,是临床诊断右冠状动脉左室瘘的首选方法。Objective To assess the pathologic anatomy features of right coronary artery fistula to left ventricle and the diagnostic value of echocardiography. Methods One adult with right coronary artery fistula to left ventricle who admitted to Qingdao Municipal Hospital (Group) on May 14,2015 and 31 cases who reported by literatures were enrolled in this study. The echocardiographic characteristics and echocardiographie diagnostic value of right coronary artery left ventricular fistula were analyzed.Results All 32 cases were confirmed by surgery or angiography, including 29 cases of surgical cases.Twenty-nine cases were diagnosed by preoperative echocardiography .the diagnosis coincidence rate was 90.63% ;Two case were diagnosed by preoperative uhrasonography (not clear) combined with imaging diagnosis. One case was misdiagnosed as sinus of valsalva aneurism rupture,the misdiagnosis rate was 3.12%.Conclusion Echocardiography can real-time dynamically display pathological anatomy of right coronary artery to left ventricular fistula and hemodynamic changes dynamically noninvasivcly, quickly, efficiently with high, accurately.h is the preferred method in the diagnosis of right coronary artery left ventricular fistula.
