医疗服务价格调整对县级公立医院收支结构的影响评价——以遵义市为例

摘　　要：通过比较遵义市8所县级公立医院在医疗服务价格调整前后的医院收入与费用结构,分析医疗服务价格调整对县级公立医院收支结构的影响,为遵义市医疗服务价格调整政策的顺利实施与进一步完善提供参考依据。研究结果显示,在医疗服务价格调整后,医院总收入增长的同时,药品收入在门诊和住院两大业务中却有不同程度的降低,医务性收入在门诊和住院服务中均提高,检查性收入在门诊中提高而在住院服务中持平。从费用结构来看,门诊次均费用提高了13%,住院次均费用持平,药品费用在门诊和住院服务中均有大幅度的下降。由此可见,遵义市现行的县级公立医院医疗服务价格调整政策在短期内能在一定程度上抑制医疗费用的不合理增长,从长远来看,需要政府加大政策执行和监督力度,才能真正破除"以药养医"局面。To analyze the influence of Medical Service Price Adjustment on hospital＇ s income and expenditure structure, the structure of the income and expenditure of 8 county public hospitals in Zunyi City before and after the price adjustment were compared, which would give some reference for the implement and development of the Medical Price Adjustment mechanism. The results show that, after the medical price adjustment the total income of hospitals have been increased, however, the drug revenue decrease observably in both inpatient and outpatient care. Of the total income, the medical services income of inpatient and outpatient care increase, meanwhile, the inspection income increase of outpatient care and keep even of inpatient, respectively. From the expenditure structure, the expenditure of outpatient and inpatient per time increase by 13% and 0, respectively, while the expenditure of drugs decrease observably. It illustrates that the Medical Price Adjustment in Zunyi City remarkably prevent the Medical expenditures from unreasonably increasing to some extent in the short period, however, in the long run the government still need strength the policy implement and supervision enforcement, in order to solve the troublesome of ‘drug oriented medical service＇.
