锁定加压钢板和普通T型钢板治疗桡骨远端die-punch骨折的研究

摘　　要：目的研究锁定加压钢板和普通T型钢板治疗桡骨远端die-punch骨折疗效和愈合情况。方法选取2006年1月~2015年10月收治的60例桡骨远端die-punch骨折患者,根据手术方法分为观察组和对照组。观察组应用锁定加压钢板治疗,对照组应用普通T型钢板进行固定治疗。分析两组患者术后骨折愈合情况及疗效。结果观察组的腕关节尺偏活动度、掌倾角、尺偏角恢复情况[(21.8±3.7)°、(9.4±2.7)°、(21.3±3.8)°]明显优于对照组[(17.8±3.1)°、(8.1±2.0)°、(18.4±3.8)°,P<0.05]。术后,观察组患者的腕关节恢复优良率(90.0%)明显高于对照组(66.7%),两组比较有统计学意义(χ2=4.8118,P=0.0283)。观察组患者并发症发生率(6.7%)明显低于对照组(20.0%),两组比较有统计学意义(χ2=4.3200,P=0.0377)。结论锁定加压钢板治疗桡骨远端die-punch骨折疗效显著,愈合情况良好,值得推广。Objective To study the effect and healing of locking compression plate and T plate in the treatment of die-punch fracture of distal radius.Methods Sixty patients with die-punch fracture of distal radius in our hospital from Jan.2006 to Oct.2015 were studied.They were divided into the observation group and the control group according to the operation method.Patients in the observation group were treated with locking compression plate fixation,and the control group was treated with T plate fixation.After operation,the fracture healing of two groups were analyzed.Results The wrist ulnar deviation activity,palmar tilt,ulnar deviation recovery of the observation group（21.8 ± 3.7°,9.4 ± 2.7°,21.3 ± 3.8°） were significantly better than those of the control group（17.8 ± 3.1°,8.1 ± 2.0°,18.4 ± 3.8°,P〈0.05）.After surgery,wrist joint recovery rate of patients in the observation group（90%） was significantly higher than that of the control group（66.7%）,and the difference was statis-tically significant（χ^2= 4.8118,P = 0.0283）.The complication rate of the observation group（6.7%） was significantly lower than that of the control group（20%）,and the difference was statistically significant（χ^2= 4.3200,P =0.0377）.Conclusion Locking compression plate in the treatment of die-punch fracture of distal radius has significant curative effect,and the healing condition is good.
