施维群辨治慢性乙型肝炎经验

摘　　要：认为慢性乙型肝炎涉及络分、血分,涵盖三阴经肝脾肾三脏,病邪已不单是疫气,还夹杂了湿、痰、瘀血等,病位由气分深入血络。治疗大法以扶正化恶血为主,以养肝益肾、疏肝健脾化瘀、滋阴益气化恶血为主要治疗方法。方药多化裁于经方,如平调寒热虚实之乌梅丸,开郁疏肝之四逆散,调和肝胃之柴平散,扶正化瘀之鳖甲煎丸,行气化瘀之旋覆花汤,健脾益肾渗湿之参苓白术散,调和肝脾之逍遥散等。并自拟养肝益肾、健脾清透的养肝益肾方。This paper believed that the locations of chronic hepatitis B involved collateral phase and blood phase,three yin meridians including the liver,spleen and kidneys.The pathogens included not only the pestilence but also mixed with dampness,phlegm and blood stasis,etc.from qi phase into blood collaterals.The treatment principles was based on strengthening the body resistance and resolving extravasated blood,including nourishing the liver and kidneys methods,soothing the liver qi and invigorating the spleen methods,nourishing yin,tonifying qi and resolving extravasated blood methods.Formulas modified from classical formulas such as Wumei pills（regulating cold and heat,deficiency and excess）,Sini powder（soothing stagnated liver qi）,Chaiping powder（regulating the liver and stomach）,Biejia Jian pills（supplementing the healthy qi and promoting blood circulation）,Xuanfu Hua decoction（promoting the circulation of qi and blood）,Shen Ling Baizhu powder（invigorating the spleen and eliminating the dampness） and Xiaoyao powder（regulating the liver and spleen） were used by adding and subtracting.Yanggan Yishen decoction was also used to nourish the liver and kidney and invigorate the spleen.
