县级医院住院病人常见病、多发病病种界定分析

摘　　要：目的:框定贵州省县级医院住院病人常见病、多发病病种范围,为划分贵州省县级医院基本医疗病种范围提供数据参考。方法:收集贵州省研究地区2012-2014年县级医院全部新农合住院患者农合报销病种相关数据,以国际疾病分类ICD-10为标准进行病种分类,按病种人次合计数进行降序排序,按ICD编码前3位进行大类合并,合并后取占人数前90%病种作为病种最后研究范围,并对病种的构成进行分析。结果:得到143种常见、多发病病种,主要以呼吸系统、消化系统、泌尿生殖系统疾病为主,分别占21.03%、15.38%、14.61%;这些病种在县内外医疗机构均有分布,其中县外就诊率为32%,县内就诊率为68%。结论:贵州省县级医院住院病人常见、多发病病种为143种,在县外医疗机构均有分布,病人无序就医仍广泛存在,县级医院诊疗能力需继续加强,重点从常见病、多发病着手,做好守门人的角色,完善基本医疗病种覆盖,早日能达到将90%县域内常见、多发病留在县内的要求。Objectives：To define the common and frequency disease in Guizhou and give evidence for basic disease admission in county level hospital in Guizhou. Methods：The data of the NCMS compensation cases were collected from 2012 to 2014 in the county-level hospitals in Guizhou province. The classification of diseases was classified according to ICD-10（International Classification of Diseases）. The total number of the total number of people sorted in descending order, according to the ICD code before the three major categories of mergers, the merger took the first 90% of the number of patients as the final scope of the disease, and disease composition analysis. Results：There were 143 kinds of common diseases, mainly respiratory, digestive and urogenital diseases, accounting for 21.03%, 15.38% and 14.61% respectively. The diseases were distributed in medical institutions both inside and outside the county. Among them, the rate of attending the prefecture is 32%, and the rate of attending the county is 68%. Conclusions：There are 143 kinds of common diseases and frequently-occurring diseases in the inpatients of county level hospitals in Guizhou Province. They are distributed in the medical institutions outside the county, and disorderly medical treatment is still widespread. The diagnosis and treatment capacity of county hospitals should be strengthened. Disease-prone to do a good job of the role of the gatekeeper, improve the coverage of basic medical diseases, as soon as possible to reach 90% of the common disease in the county to stay in the county＇s frequently asked disease.
