摘 要:目的探讨通心络对心肌梗死模型大鼠左心室结构的改善及其相关机制。方法采用结扎前降支方法制作心肌梗死大鼠模型,随机分为模型组、通心络组、培哚普利组,每组10只;假手术组9只仅穿线不结扎。药物组大鼠分别以通心络、培哚普利灌胃,模型组和假手术组给予等量羧甲基纤维素钠溶液灌胃。2周后测量左心室梗死区面积、梗死区最小厚度、室间隔厚度,免疫组化技术测量转化生长因子-β1(TGF-β1)、结缔组织生长因子(CTGF)表达水平。结果通心络、培哚普利可改善模型大鼠的一般情况。梗死面积:通心络组、培哚普利组小于模型组,大于假手术组(P<0.01)。室间隔厚度:模型组较假手术组增大(P<0.05),但模型组、通心络组、培哚普利组组间差异无统计学意义(P>0.05)。梗死区最小厚度:培哚普利组较模型组增厚(P<0.05),通心络组、培哚普利组较假手术组减薄(P<0.01)。梗死区与室间隔厚度比:通心络组、培哚普利组高于模型组(P<0.01),低于假手术组(P<0.01)。TGF-β1与CTGF:通心络组、培哚普利组低于模型组(P<0.05,P<0.01),高于假手术组(P<0.01)。CTGF:通心络组低于培哚普利组(P<0.01)。结论通心络可通过减少TGF-β1、CTGF的表达抑制心肌纤维化,改善模型大鼠左心室结构,保护心脏功能。Objective To discuss the improvement of left ventricular structure in myocardial infarction model rats by Tongxinluo and its mechanism. Methods The rat model with acute myocardial infarction was established by ligaturing the left anterior descending coronary artery. 30 model rats were divided randomly into the model group, Tongxinluo group, Perindopril group of 10 in each group. And another 9 rats in sham operation group were punctured only without ligation. The rats in each medication group were garaged respectively by Tongxinluo suspension and Perindopril suspension with equal dosage. And the model group and the sham operation group were gavaged by sodium carboxymethyl cellulose solution with corresponding equal dosage. After 2 weeks, the area, minimum thickness of myocardial infarction and interventricular septal thickness were measured. The level of TGF-131 and CTGF was detected by IHC with corresponding antibodies. Results Tongxinluo and Perindopril therapy could improve the general condition of rats. Myocardial infarction area: The Tongxinluo group and Perindopril group were less than the model group and larger than the sham operation group (P 〈 0. O1 ). Interventricular septal thickness : the model group was thicker than the sham operation group ( P 〈 0. 05 ), but the differences among the model group, Tongxinluo group and Perindopril group were not statistically significant ( P 〉 O. 05 ). Minimum thickness of myocardial infarction : Perindopril group was more thickened than the model group ( P 〈 0. 05 ), Tongxinluo group and Perindopril group were much thinner than the sham operation group ( P 〈 O. 01 ) . Thickness ratio of myocardial infarction and interventricular septal: Tongixnluo group and Perindopril group were higher than the model group( P 〈 0.01 ), while lower than the sham operation group (P 〈0. 01 ).
