摘　　要：目的:分析评价社区脑卒中后遗症患者康复效果,探索家庭医生社区服务的新模式。方法:以社区脑卒中后遗症患者为研究对象,采用随机分组对照干预研究方法,分析比较干预组和对照组患者"自测健康评定量表"(SRHMS)得分和社会经济效益,以及研究对象接受服务的满意度。结果:本次研究对象共176人,在接受服务六个月后,干预组"自测健康评定量表"(SRHMS)评估结果显示生理健康、心理健康、社会健康子量表得分和总得分均优于对照组(P<0.01);月均就诊次数、就诊时间和家人日均护理时间减少,医药费用下降(均为P<0.01),同时,研究对象对家庭医生总体评价满意度为97.73%。结论:家庭医生签约服务模式对脑卒中后遗症患者有较好的康复效果,具有较好的社会效益和经济效益,受到社区居民欢迎,值得推广。Objective： To evaluate the impact of contract service provided by GP on the patient with stroke sequelae, to explore new method of community health service. Methods： 176 patients with stroke sequelae were devided into intervention group and control group equally. The score of SRHMS of intervention group patients has been compared with that of control group, while the social and economic benefit of contract service, satisfaction of the patients has been analyzed. Results： 88 patients have received contract service provided by GP for 6 months. The total score of SRHMS and each score of 3 sub - scales of SRHMS are apparently higher than those of control group（ P 〈0. 01 ）;monthly average visiting time, frequency, medical costs and daily average cared time are decreased （P 〈 0. 01 ）. Otherwise, the satisfaction score of the patients to contract service is 97.73%. Conclusions： The contract service provided by GP can improve rehabilitation of the patient with stroke sequelae. There is a well social and economic benefit produced by contract service. It is worth spreading.
