经食管超声心动图引导下经胸微创封堵术治疗婴幼儿室间隔缺损的有效性和安全性

摘　　要：目的探讨经食管超声心动图(TEE)引导下经胸微创封堵术治疗婴幼儿室间隔缺损(VSD)的有效性和安全性。方法选取2015年5—8月青海省人民医院收治的婴幼儿VSD 26例。患儿均在TEE引导下行经胸微创封堵术,术后1、3个月门诊随访。结果 26例患儿成功完成TEE引导下经胸微创VSD封堵,2例患儿术中发现微量残余分流,3例患儿术后3d复查出现少量胸腔积液,术后患儿均顺利康复出院。术后1个月复查超声心动图2例患儿残余分流均消失,3例患儿胸腔积液完全吸收。术后3个月复查均无封堵器脱落、无房室传导阻滞。结论 TEE引导下行经胸微创封堵术治疗婴幼儿VSD是安全、有效、可行的。Objective To investigate the efficacy and safety of transthoracicminimally invasive occlusion in the guidance of transthoracic echocardiography（ TEE） treated on ventricular septal defect（ VSD） infants. Methods A total of 26 cases of VSD infants were selected from May to August 2015 in Qinghai Provincial People＇ s Hospital, all infants were given transthoracicminimally invasive occlusion in the guidance of TEE,followed-up after operation of 1,3 months,evaluated the efficacy with TEE. Results 26 cases of infants were successfully completed VSD closure by transthoracic,2 cases were found in residual shunt,3 cases of postoperative 3 days there was a small amount of pleural effusion, postoperative patients were successfully discharged in January. Postoperative echocardiography 2 cases of residual shunt disappeared,3 cases of pleural effusion completely absorbed. After March was no occluder detachment, no atrioventricular block. Conclusion TEE guidedminimally invasive percutaneous closure of VSD in infants and young children is safe,effective and feasible.
