摘　　要：目的:探讨鼻内镜手术治疗真菌性鼻-鼻窦炎(FRS)的疗效及对鼻通气功能、血液学指标的影响。方法:88例FRS患者经随机数表法分为观察组(n=44)及对照组(n=44)。对照组接受传统手术治疗,观察组接受鼻内镜手术治疗。术后1周,采用主观评价量表鼻腔鼻窦结果测量20条(SNOT-20),客观评价量表Lund-Mackay鼻窦CT评分、Lund-Kennedy鼻内镜评分评估手术治疗效果;采用鼻阻力仪检测鼻通气功能;采用酶联免疫吸附法检测血清炎症因子含量。结果:手术后1周,观察组的SNOT-20评分、Lund-Mackay评分、Lund-Kennedy评分值明显低于对照组,P<0.05;观察组的鼻腔容积、鼻呼吸量、鼻腔最小横截面积水平明显高于对照组,鼻通气阻力水平明显低于对照组患者,P<0.05;观察组的血清白介素-6、白介素-8、白介索-12、白介素-17含量显著低于对照组,P<0.05。结论:鼻内镜手术可有效优化FRS患者的疗效,在增加鼻通气、增强免疫功能的同时缓解全身炎症反应。Objective：To study the effect of endoscopic surgery in fungal rhinosinusitis patients and its influence on nasal ventilation function,haematological indices.Methods：88 cases of fungal rhinosinusitis patients in our hospital during February 2013-February 2016 were choosed as research object and divided into observation group（n =44） and control group（n =44） according to random data form method,each group had forty-four cases in it Patients in control group received traditional operation and those in observation group received nasal endoscopic surgery.One week after surgery,Surgical treatment effect was evacuated by subjective scale of the sino-nasaloutcome test-20（SNOT-20）,objective scale Lund-Mackay sinus CT score,Lund-Kennedy score;Nasal ventilation function was tested by nasal resistograph;serum inflammatory factors were detected by Enzyme linked immunosorbent assay（ELISA）.Results：One week after surgery,patients ＇ SNOT-20 score,Lund-Mackay score,Lund-Kennedy score were lower in observation group than in control group,the difference was statistically significant（P〈0.05）;patients＇ nasal ventilation function indices NMCA,NCV,NS were higher in observation group than in control group,NAR level was lower in observation group than in control group,the difference was statistically significant（P〈0.05）;patients＇ serum IL-6,IL-8,IL-12,IL-17 were lower in observation group than in control group,the difference was statistically significant（P〈0.05）.Conclusion：Endoscopic surgery can effectively optimize the efficacy,increase nasal ventilation,enhance immune function in fungal rhinosinusitis patients,and ease the systemic inflammatory response.
