静脉用丙种球蛋白联合甲泼尼龙冲击治疗免疫性血小板减少症的疗效观察

摘　　要：目的探索静脉用丙种球蛋白联合甲泼尼龙(甲强龙)冲击治疗免疫性血小板减少症(ITP)的临床疗效。方法将92例ITP患儿按随机数字表法分为两组,对照组(46例)给予甲强龙治疗,观察组(46例)给予甲强龙联合丙种球蛋白冲击治疗,比较临床治疗效果。结果观察组有效率(89.13%)较对照组(73.91%)明显提高,差异有统计学意义(Z=5.83,P<0.01)。观察组患儿治疗后血小板计数(122.45±12.63)×10~9·L^(-1)较治疗前(12.04±1.50)×10~9·L^(-1)和对照组同期(71.54±7.34)×10~9·L^(-1)均有明显增加(P<0.05)。患儿主要不良反应为柯兴综合征面容、高血压、高血糖、恶心、头痛等,两组不良反应发生率比较差异无统计学意义(P>0.05)。观察组复发率(4.35%)明显低于对照组(17.39%),差异有统计学意义(P<0.05)。结论静脉用丙种球蛋白冲击治疗ITP可明显改善患儿临床症状,提高外周血中血小板计数,降低复发率,具有较高的临床应用价值。Objective To explore the clinical efficacy of intravenous gamma globulin combined with methylprednisolone Impact treat-ment of Immune thrombocytopenia （ITP）. Methods 92 cases of ITP children were divided into two groups according to the random number table method. The control group （46 cases） were treated with methylprednisolone. The observation group （46 cases） were trea-ted with methylprednisolone combined with gamma globulin. The clinical treatment effect were compared. Results The effective rate of the observation group （89. 13% ） was significantly higher than that of the control group （73. 91% ） ,the difference was statistically sig-nificant （ Z = 5. 83,P 〈 0. 01）. The platelet count in the observation group （ 122.45 ± 12. 63 ）×10^9·L-1 was significantly higher than that before treatment （ 12. 04 ± 1.50）×10^9·L-1 and the control group （ 71.54 ± 7. 34 ）×10^9·L-1 （ P〈0.05 ） . The main adverse reactions were Cushing syndrome face,high blood pressure,high blood sugar,nausea,headache,etc. ,There was no significant difference between the two groups （P 〉0. 05）. The recurrence rate （4. 35% ） of the observation group was significantly lower than that in the control group （17.39%） ,the difference was statistically significant （P〈 0.05 ） . Conclusion Intravenous administration of gamma globulin in the treatment of ITP,which can significantly improve the clinical symptoms of children,improve platelet count in peripheral blood,reduce the recurrence rate,has a profound clinical value.
