透明帽辅助胃镜治疗贲门息肉的疗效研究

摘　　要：目的探讨透明帽辅助胃镜治疗贲门息肉的临床疗效。方法选择收治的贲门息肉患者共48例,随机分为两组,各24例,其中研究组采用透明帽辅助胃镜下治疗,对照组采用普通胃镜下治疗。观察并记录病变的暴露清晰度、定位难度、切除时间、并发症发生的情况与总操作时间。结果研究组病人中操作暴露清晰(95.83%)与定位准确(91.66%)的比率高于对照组(79.17%,75.00%),但差异无统计学意义(P>0.05)。研究组息肉切除所需时间(18.34±5.62 min)和总操作所需时间(33.42±9.39 min)显著低于对照组(23.45±6.12 min,41.34±10.56 min),差异均有显著性(P<0.05)。研究组出现1例黏膜轻微擦伤的病人,对照组出现4例少量出血和6例黏膜轻微擦伤,两组均未出现黏膜穿孔和死亡病例。研究组发生少量出血(0)和黏膜轻微擦伤的病人比率(4.17%)显著低于对照组(16.67%,25.00%),差异均有显著性(P<0.05)。结论胃镜下透明帽辅助处理贲门息肉操作简捷、安全、疗效好,实用性强,并发症少,值得临床应用。Objective To explore the effect of transparent cap assisted gastroscopy in treatment of cardiac polyposis. Methods 48 patients with cardiac polyposis were collected and randomly divided into two groups,24 cases in each group. Patients in study group were treated by transparent cap assisted endoscopy,and patients in control group were treated by general gastroscopy. The degree of clearness of lesions,the difficulty of localization,the duration for resection,the occurrence of complications and the total operating time were observed and recorded. Results There was a greater proportion of patients exposed to clear exposures（95.83%） and more accurate positioning（91.66%） in study group than those of control group（79.17%,75.00%）,but the difference was not statistically significant（P〉0.05）. The duration for complete polypectomy（18. 34 ± 5. 62 min） and total time to complete the procedure（33.42 ± 9.39 min） were significantly lower（P〈0.05） in study group thanthose of control group（23.45 ± 6.12 min,41.34 ± 10.56 min）. In study group,there was 1 patient with mild mucosal abrasion,and 4 patients with minor hemorrhage,and 6 patients with mild mucosal abrasion in control group. No mucosal perforation and death occurred in these two groups. The proportion of patients with minor hemorrhage（0） and mucosal abrasion（4.17%） in study group（16.67%,25.00%） was significantly lower than that of control group（P〈0.05）. Conclusion The application of transparent cap assisted gastroscopy in treatment of cardiac polyposis is a simple,safe,effective and practical method for treatment of cardiac polyposis,and it is worthy to be clinically applied.
