膀胱瘤样病变内囊泡状结构的超声诊断学意义探讨

摘　　要：目的通过对比膀胱瘤样病变中囊泡状结构的超声表现及病理学图像基础,探讨膀胱瘤样病变内囊泡状结构的超声诊断学意义。方法回顾性收集2010年12月至2013年5月收治的347例膀胱瘤样病变患者超声声像图,筛选出具有囊泡状结构的病灶18例,并进行了一一对应的病理图像对照研究。结果 18个膀胱瘤样病变中囊泡状结构均为多个,超声表现为类圆形无回声结节,直径0.2~0.7 cm,边界清楚,形态规则,分布于团块的内部及表面,内部无血流信号。而各种病理类型中的囊泡状结构超声表现并无明显不同。347例膀胱瘤样病变患者中,囊泡状结构发生率为5.19%。18例具有囊泡状结构患者中,10例为膀胱尿路上皮癌(其中7例为低级别,3例为高级别),3例为肉芽肿性炎伴上皮增生,3例为腺性膀胱炎,1例为结核性肉芽肿炎,1例为内翻性乳头状瘤。结论膀胱瘤样病变内囊泡状结构可出现于多种病理类型的膀胱病变中,并非腺性膀胱炎的特征表现,而各种病理类型中的囊泡状结构在超声表现上并无显著差异。Objective To discuss ultrasound diagnosis value of vesicles in tumor - like lesion of bladder by comparing with pathological characteristics of vesicles. Methods The ultrasonographic findings of 347 patients with bladder tumor - like lesions from December 2010 to May 2013 were retrospectively analyzed. Among all the cases, vesicles existed in 18 cases. Results 18 cystic tumor - like lesions are multiple vesicu-lar stmctm＇e,the performance of ultrasound - like round non - echo nodules,diameter were 0. 2 ？ 7 cm, clear boundary,regular morphology, distributed in the clumps of the internal and surface, with no internal blood flow signal. The ultrasonographic features of vesicular structures in va-rious pathological types were not significantly different. 18 cases （incidence rate was 5.19%） with vesicles were found to be 5 different kinds of bladder diseases, 10 cases were urothelial carcinoma （7 low grade cases and 3 high grade ones） , 3 cases granuloma with epithelial hyperplasia, 3 cases cystitis glandularis, 1 case tuberculosis granuloma, 1 case inverted papilloma. Conclusion The vesicular structures of bladder tumor - like lesions can be found in a variety of pathological types of bladder lesions, and are not glandular cystitis characteristic manifestations, and various pathological types of vesicular structure in the ultrasound performance are not significantly different.
