益肾通淋方防治肾结石ESWL术后肾脏生物学效应损伤30例

摘　　要：目的:研究肾结石ESWL术后肾脏生物学效应的损伤及中药益肾通淋方药对其防治作用。方法 60例肾结石ESWL术患者随机分为两组,碎石后常规治疗为适当多饮水、抗感染、体位排石和对症处理,观察组ESWL术前3d及术后7d服用中药益肾通淋方药。两组分别于ESWL术前、术后24h、72h、168h检测尿NAG、γ-GT,血清β2-MG、NAG、CysC变化。结果 ESWL术后24h两组尿NAG、γ-GT、血清β2-MG、NAG、CysC均明显升高,与ESWL术前比较,P﹤0.05或0.01;ESWL术后24h、72h观察组尿酶、血清酶值均明显低于对照组,P﹤0.05或0.01,尤其是ESWL术后72h观察组尿、血清酶值即恢复正常;两组ESWL术后168h尿酶均恢复到正常。结论肾结石ESWL术可引起肾脏生物学效应的损伤,但在一定的时间与范围内是可逆的;中药益肾通淋方药对肾结石ESWL术后肾脏生物学效应的损伤具有快速、较好的防治作用。Objective：To research on calculus of kidney ESWL surgery of biological effects of renal injur prevention and treatment the kidney with Yishen Tanglin decoction.Methods： 60 cases of ESWL were randomly divided into two groups, the control group was under routine treatment like adequate drinking water, anti-infective therapy, postural therapy and symptomatic treatment after lithotripsy.The observation group took Yishen Tonglin Decoction 3 days before and 7 days after ESWL surgery.The changes of urinary NAG, gamma-GT, serum β2-MG, NAG and CysC were detected before ESWL and 24 hours, 72 hours and 168 hours after operation in the two groups.Result： Compared with preoperative ESWL, 24 hours after ESWL in two groups of urine NAG, serum-GT, gamma β2-MG, NAG, CysC obviously rise, P＜ 0.05 or 0.01;24 hours and 72 hours after ESWL, the serum enzyme levels in the observation group were significantly lower than those in the control group, P＜0.05 or 0.01.In particular, 72 h after ESWL treatment, the urine and serum enzyme levels were recovered to normal;In the two group, the urine enzyme recovered to normal at 168 hours after ESWL.Conclusion：Calculus of kidney ESWL can cause kidney damage, while the damage is reversible in a certain period of time and scope;TCM prescription of tonifying the kidney and treating stranguria has a fast and good the prevention and treatment affection on Kidney stones ESWL.
